The staff file for Staff person #2 was observed to have a current health assessment on file dated 6/2/25 that lacked examination for communicable diseases and the results of that examination.
Upon review of the facility's 30-day fire testing documentation, the following tests were observed to have been conducted more than 30 days apart: 6/12/25 to 7/14/25 (32 days).
In the Preschool Room an electrical outlet located between two shelving unit located to the left of the doorway, and accessible to the preschool age children in care, was observed to be missing one protective cover.

The file for Staff person #1 was observed to contain a Child Abuse clearance on file dated 1/16/24 which was not obtained prior to working with children and a State Police clearance dated 1/17/24 and there was no documentation of having applied for the State Police clearance prior to working with children (See IS CODE SHEET) as required for provisional hiring under the CPSL.
In the Toddler Room, a cleaning solution containing 70% alcohol and baby lotion labeled 'Keep Out of Reach of Children' were observed to be on the changing table located at the back of the room and considered accessible to the older toddlers in that room.

At the onset of supervision/ratio check, when asked to name the children for whom they were responsible for Staff person #1 identified and named 6 children, Staff person #5 identified and named 4 children, omitting 1 child. When staff were asked a second time to identify and name who they were responsible for Staff person #1 identified and named 6 children, Staff person #5 identified and named 4 children, again omitting 1 child. When staff were asked a third time to identify and name who they were responsible for, all children were accounted for. At approximately 10:35am in the Preschool/School Age Room 2 staff and 15 children were observed. At the onset of supervision/ratio check, when asked to name the children for whom they were responsible for Staff person #3 identified and named 5 children, Staff person #4 identified and named 8 children, omitting 2 children. When staff were asked a second time to identify and name who they were responsible for, Staff person #3 identified and named 4 children, Staff person #4 identified and named 9 children, again omitting 2 children. When staff were asked a third time to identify and name who they were responsible for all children were accounted for.
