On 7/19/24, at time of renewal, the EpiPen belonging to Child #1 was not in the original container and did not contain written instructions.
On 7/19/24, at time of renewal, emergency contact information was not present in the lower-level childcare space where 12 children were receiving care.
On 7/19/24, at time of renewal, two azalea bushes known to be toxic were observed accessible to children on the outdoor play space. Additionally, air fresheners instructing to "keep out of reach of children" were observed accessible to children in the bathroom.
During the renewal inspection on 06/09/23, a sign on which the handwashing requirement is written was not posted at the kitchen sink area.

On 9/22/2022, children were observed coming inside from outdoor play. They were seated and hand sanitizer was used on their hands instead of hands being washed. They were then served snack.
On 7/12/22, staff person #1 was observed working with children and was being counted in the staff/child ration. When asked for their file, the facility provided a child abuse clearance, undated disclosure statement, CDA certificate, pediatric 1st aid/CPR training, and fire safety training. Staff person #1 did not have a complete file that qualified them to work with children and be counted in the ratio.
On 7/12/22, staff person #1 was observed working with children on the playground and was being counted in the staff/child ration. They were working with staff person # 2 with 14 preschool children. When asked for their file, the facility only provided a child abuse clearance and undated disclosure statement. Staff person #1 did not have a complete file that qualified them to work with children and be counted in the ratio. Also on 7/12/22, staff person #3 came outside to talk to the inspector and supervisor on the playground during the visit. When we went inside the facility, we observed 14 YSA children with staff person #4. Staff person #3 told us they were assigned to that group. They had gone out of ratio when staff person #3 left the group to go outside.
On 10/7/21, during an on site inspection of the facility, five older toddlers were observed playing on the outside play space which contains mulch.
Certification representative reviewed documents for the renewal inspection on 9/13/21 (staff #1's file was emailed), 9/16/21 (staff #2, 3, & 5 files were emailed), and 9/19/21 (staff #4 file was emailed). All staff documents were submitted to cert rep via email by LE representative, staff person #6. After review it was found that Staff #1's State Police clearance dated 7/15/2020 was altered; Staff #2's Child Abuse clearance dated 10/26/2020 was altered; Staff #3's Child Abuse clearance dated 4/26/2020 was altered; Staff #4's FBI clearance dated 9/1/2021 was altered; and Staff #5's Child Abuse clearance dated 11/30/2020 was altered. These clearances were confirmed to be fraudulent/altered.
