On 4-30-26 Staff # 1 was trying to stop the child from running around the classroom, the alleged perpetrator reached out her arm and pushed the child in the face.

On 8/23/24 at unannounced inspection certification representative confirmed a self-reported non-compliance event that occurred on 8/14/24. Staff person # 1 left child # 1 unattended in a car seat for approximately one minute.
On 3/13/24 staff #1 was in the bathroom with some children while one of her children was in the hallway and eloped with another classroom that was walking by and went with them to the bathroom. Child left her group with staff #1 and went with the other classroom to the gym out of staff #1 supervision group.
On 12/16/23 at complaint investigation certification representative that staff verified staff person # 1 put a baby wipe in the child's mouth to keep the child from crying during a diaper change.

At approximately 4:30 PM on 9/29/22 an incident occurred in which Child #1 was left inside the school unsupervised, while the class was playing on the playground. According to our security cameras, Staff person #1 can be seen taking two students, including child #1 towards the preschool classroom playground door to use the bathroom at 16:30. Staff person #1 can be seen on the video at 16:32 walking away from the door without child #1.

At the time of the inspection the emergency contact information for child #1 was not in the space they were receiving care.

At the time of the inspection there was no tape in the first aid kit the preschool room as well as toddler 2 room.
At the time of the inspection the record of staff person #1 contained and FBI clearance dated over 60 months from the date of the inspection.
At the time of the inspection the preschool group was utilizing the "all purpose" room adjacent to their official classroom. no first aid kit was in the room they were in. In the large muscle play space there was a group of older toddlers and no first aid kit was in the space where they were receiving care.
During the course of this inspection an incident involving child #1 was discovered. Child was taken to the ER and facility did notify the regional office 3 days after the incident.
At the time of the inspection the record of staff person #2, who began employment 2/22/22, had no proof of the completion of pediatric first aid and pediatric cardiopulmonary resuscitation.
