The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

At the time of the inspection the record of staff person #1 was lacking in the proof of training in the emergency plan of the facility.

At the time of the inspection staff member #1 was providing care and has been for more than 45 days at the facility. The record of staff person #3 did not have a completed disclosure statement.

At the time of the inspection the record of staff persons #2 was lacking in the proof of having completed the required health and safety training and the record of staff person #3 was lacking in the proof of the completion of the required first aid training. Both staff person #2 and #3 have been serving the facility for over 90 days.

Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times.

At the time of the inspection the children in care were observed leaving the room where they were in care, they were observed in the hallway of the facility and when inspector did interview staff regarding the fact that the children were out of their line of sight they reported "they always come back"
