On 7/17/24 during unannounced visit, teacher in young toddler room diapered a child and failed to wash the child's hands after being diapered.
On 7-17-24 during unannounced allocated monitoring Certification Representative noticed that the young toddler teacher failed to wash her hands after diapering a child.
On 7-17-24 during unannounced allocated monitoring Certification Representative noticed that there was no schedule posted in several classrooms.
On 1/30/24 during renewal inspection Certification Representative noticed that a few sippy cups in the two's room were not labeled.
On 1/30/24 during renewal inspection Certification Representative noticed that a few handwashing sigs was missing.
On 1/30/24 during renewal inspection Certification Representative noticed that items were missing from the first aid kits.
At the time of the inspection the first aid kit in the infant room was lacking in band aids.
At the time of the inspection the records of the following staff persons had areas of non compliance with regards to the CPSL requirements: #1 was lacking the National Sex Offender Clearance letter, #2 was lacking child abuse clearance, #3 had both an incomplete FBI clearance and an incomplete state police clearance, #5 had an FBI clearance that did not satisfy CPSL requirements for child care.
At the time of the inspection the records of the following staff persons had areas of non compliance with regards to qualifications: staff person #3, #4 and #6 lacking verification of experience
At the time of the inspection staff person #3 did enter the hallway while their primary care group was in the room. Some children in their primary care group were not able to be seen at that time.
At the time of the inspection the record of staff person #2, who was observed caring for children, was lacking the required Mandated Reporter Training.
At the time of the inspection the record of staff person #4 was lacking in a current health assessment.
At the time of the inspection in the room known as "the ducks" there were toxic cleaning supplies stored in an unlocked cabinet beneath the sink in an area accessible to the children.
At the time of the inspection the molding trim along the baseboard was coming off the wall, this was located in the rear corner of the room known as the "ducks"
At the time of the inspection in both of the rooms known as "the bears" and "the bumblebees" there were no lidded waste receptacles.
At the time of the inspection the record of child #3 had an emergency contact form that was lacking health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.
At the time of the inspection the record of staff person #6 had a health assessment with no tuberculosis screening by the Mantoux method or other medical documentation necessary to confirm freedom from communicable tuberculosis.
At the time of the inspection staff person #8 was observed providing care for children under the supervision of a Group supervisor level staff person. There was no staff file for staff person #8 at the time of the inspection.
At the time of the inspection the record of child #2 was lacking in a current valid health assessment.
At the time of the inspection in the room known as "the bumblebees: there was initially a group of 10 older toddlers with 2 assistant group supervisor level facility staff persons including staff person #7. Staff person #7 was observed by this writer leaving the (bumblebees) room, leaving their partner with a 1:10 ratio with older toddlers.
At the time of the inspection the records of child #1 and child #2, who were both in care on the date of the inspection, were lacking in the consents required prior to admission.
At the time of the inspection staff person #1 was the only staff person providing care for a group of older and younger toddlers at the facility.
