Parents of Child 1 took child 1 to the emergency room, after school due to reaction at school. Parents notified school; however, daycare center did not notify the regional office that the child went to Hospital ER within the 24-hour period.

At the time of the inspection, Staff 1 and Staff 3 did not have a health assessment on file.
At the time of the inspection, Staff 2 did not have proof of New Jersey State Clearances and has lived in NJ in the last 5 years. Staff 3 did not have proof of their NSOR clearance. Staff 2 and Staff 3 do not qualify as provisional hires.

Certification rep observed a four year old child walk out of the the pre k class and down the hallway to use the bathroom unsupervised while staff #1 was inside the classroom.Certification rep observed a two year old toddler inside a playpen in the hallway to the left of the doorway entrance of the older toddler classroom unsupervised, while staff#2 was inside the classroom reading to the other children present.
Certification rep reviewed child #5 file and observed enrolling parents work address and work phone number are missing on the emergency contact form.
Certification rep observed refrigerator thermometer temperature read 50 degrees in the nursey room refrigerator.
Staff person #4,6,11 and 12 have not completed the full 12 clock hours of child care training.
During renewal inspection, certification representative observed unpainted plaster on the right, middle and left corner of the wall of the entry way of the classroom on the second floor.
Staff person #5 has acceptable pediatric first aid/CPR training in an approved curriculum; however, the trainer has not provided documentation that they are PQAS-certified in that training topic.
STAFF 1 DID NOT HAVE PROOF OF THE FOLLOWING CLEARANCES NOT PROOF THAT THEY APPLIED - FBI AND NSOR - HIRED 9/7/21. STAFF 2 WAS LISTED AS STAFF BUT HAD VOLUNTEER STATUS STATE POLICE AND CHILD ABUSE CLEARANCES AND DID NOT HAVE PROOF OF THEIR NSOR. STAFF 3HAD THE RECEIPT FOR THE STATE POLICE CLEARANCE BUT NOT THE ACTUAL FORM AND IS CONSIDERED STAFF HIRED 01/2020.
CHILD 2 DID NOT HAVE PROOF OF CONSENT FOR EMERGENCY MEDICAL CARE IN THEIR FILE NOR ON THE EMERGENCY CONTACT FORM.
STAFF 1 HAD ONLY 1 LETTER OF REFERENCE IN THEIR FILE.
During renewal inspection, certification representative observed toxics (Mr. Bubbles cleaning spray) in the bathroom in an area accessible to children in the pre-school room.
Certification observed there was not a complete first aid kit on the personal vehicle that transports the children to and from school. The first aid kit was missing soap, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.
Certification rep observed pea gravel outside on the playground less than one inch in size. During the renewal inspection, certification representative observed the facility climbing equipment in their play space. The equipment was not mounted over enough protective surfacing covering to meet the requirements of the USCPSC, the pea gravel and ripped black tarp had grass growing through it.
Certification rep reviewed child#1 file and observed enrolling parent home phone number was missing on the emergency contact form and operator signature was missing on the fee agreement form
Certification rep reviewed child#3 file and observed the Childs date of admission was missing on the fee agreement form.
Certification rep reviewed child #4 file and observed extra services was left blank and consent on financial agreement form and parent consent was missing for walk and trips on the emergency contact form.

Certification rep reviewed Child#3 file and observed financial agreement was not updated within a six month period. Parental consent was observed missing for swimming and wading on the emergency contact form. Child #4 was missing parental consent for walking and excursion's on the emergency contact form.
Certification rep reviewed child #4 file and observed name, address and telephone number of the child's physician or source of medical care are missing on the emergency contact form.
During the renewal inspection, certification representative observed two broken ceiling tile in the pre k classroom. The ceiling tile next to the broken tile has water damage in addition, to the stairwell to the second floor and the entrance to the pre k classroom.
Staff person#4,6,7 and #8 has not completed mandated reporter training within the last 60 months. This is evidenced by the previously documented mandated reporter training on file being dated for staff# 4: 8/17/17, staff#6 9/23/17, staff#7 11/12/16 and staff#8 4/26/17.
Staff#1,2 and #4 physical examination are expired including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Staff #1,health assessment form on record is from 9/30/20 TB test from 6/26/17,hire date is 4/9/19. Staff #2,health assessment form on record is from 10/10/19 TB test from 10/10/19 hire date: 6/1/20. Staff#4 health assessment form on record is from 6/5/20 date of hire 9/1/17 .
Staff#10 hire date 8/12/21 does not have NSOR certificate on record. Staff#9 hire date:11/2/21 does not have NSOR on record was observed to be working with children in the two-year-old classroom.Staff#6 clearances on record are expired state police 9/18/17child abuse9/20/17 and FBI clearance 9/20/17.Staff person #6 does not have current pediatric first-aid/CPR training. This is evidenced by the previously documented pediatric first-aid/CPR training expiration date of 9/20/17. Staff#4 hire date :9/1/17 clearances on record are expired state police 4/23/17 child abuse 5/2/17 and was working alone with children in the 3 year old room at the time of inspection.Staff#7 Staff person does not have current pediatric first-aid/CPR training on record.
CHILD 1, 2 AND 3 DID NOT HAVE PROOF OF A RECENT FLU VACCINE NOR DID IT CONTAIN A LETTER OF EXEMPTION.
