Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: 1. Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff who are involved in transporting children to/from the mode of transportation to/from classroom staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. The operator shall provide a date for when this training will be completed. 3. The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children during to/from the mode of transportation to/from classroom staff is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format. The correction date for this portion of the plan shall be a minimum 3 months from the date the Regional Office approves the checklist.
Facility staff self-reported on 5/5/2026 at approximately 8:03am child #1 (DOB see LIS code sheet) was escorted from the facility's van to the Violet's classroom by staff person #1. Staff person #1 opened the classroom door and dropped child #1 off without ensuring the transfer of supervision to another staff member. Unbeknownst to staff person #1, staff person #2 and the rest of the violet classmates were outside on the playground, which resulted in child #1 being left unsupervised in the Violet classroom until staff person #2 noticed child #1 standing by the playground door, waving at them. Staff person #2 immediately retrieved child #1 at approximately 8:05am and reported the incident to administrative staff.
