The file for Staff Person #2 contained documentation of fire safety training that was not renewed prior to the expiration date of the most current certificate (9/20/24 and 1/20/26). At the time of inspection, the file for Staff Person #3 did not contain documentation of fire safety training within the last 12 months (11/20/24).
The file for Staff Person #1 lacked a signed disclosure statement. The file for Staff Person #1, who had lived in New Jersey during the previous 5 years of employment, did not contain documentation of out of state clearances as required under CPSL to work in a child care position. In addition, Staff Person #1 was observed supervising children in Room 1 while alone and unsupervised by a permanent employee. The file for Staff Person #4 did not contain documentation of State Police application or clearance results prior to the staff person's first day working with children as required under CPSL (See LIS Code Sheet). In addition, Staff Person #4 was observed supervising children in Room 4 while alone and unsupervised by a permanent employee.
The files for Staff Persons #1 and #4, identified as assistant group supervisors, lacked qualification documents for the position in which the staff persons are performing.
In Room 4, a visual strip or visual identification was not placed in a traffic area on the glass door.

At the time of inspection, the file for Staff Person #1 did not contain documentation of two written, non-family references from individuals attesting to the person's suitability to serve as a facility person. At the time of inspection, the file for Staff Person #4 contained one written, non-family reference from an individual attesting to the person's suitability to serve as a facility person.
This change in location capacity inspection recorded no violations or advisories.
A stuffed animal with hard, plastic eyes were observed in room #1, behind the changing table with one eye not attached posing a choking hazard.
In Room #1 above the changing table, were loose push pins in the wall which pose a puncture or cutting hazard.
A copy of the facility's current Emergency Plan was not posted in a conspicuous place.
In Room #1, on the right wall, next to door #2, one protective receptacle cover was missing which was accessible to children 5 years of age or younger.
The boys and girl's restroom lacked handwashing signs above the toilets.
In the Pre-School Room, the lid for blue bin with toy figures was cracked posing a cutting hazard and/or pinch point. In the Older Toddler Room, a clear bin with large Legos was cracked posing a cutting hazard and/or pinch point.
The following unsafe condition was observed in the Indoor Gross Motor play space: Heavy, black, rubber blocks (2' x 2' x 4") used as a base for the outdoor play space was stacked and poses a toppling or crushing hazard.
Facility files lacked proof of purchase for the interconnected smoke detection system.
In private restroom, it was observed a bottle of "Windex," "Clorox Disinfecting Bathroom Bleach-Free Cleaner," "Lysol Wipes," and "AJAX Powder, "which were labeled, "Keep out of the reach of children" was accessible to children while in the restroom under the sink.
The portable sink in the Toddler Room diapering area did not have running water creating both a safety and cross-contamination concerns.
In room #4 next to the kitchen on the bottom shelf of the black, plastic shelf was a bin with stuffed animals which had a broken, green lid handle. In the Preschool Room, a plastic "Little Tykes" basketball hoop was not secured which poses a toppling hazard. In the Preschool Room, a clear, plastic bin with plastic building blocks had a broken corner posing a cutting hazard.
In Room #4, two blue, wooden child chairs with loose screws were wobbly which posed a pinch point or fall hazard. In Room #4, the changing table with loose screws was wobbly which posed a falling and/or toppling hazard. In Room #4, The "Little Tykes" play kitchen was wobbly and unsecured posing a toppling hazard. In room #4, a child chair across from the play kitchen with loose screws was wobbly which posed a pinch point or falling hazard. In Room #1, the wooden bookcase, left of the door, the corner was chipped with splintering wood which poses a splintering hazard. In Room #1, the bookcase, left of the door, was missing two screws making the bookcase unstable which posed a toppling hazard or pinch point. In Room #1, a black plastic, shelf was wobbly posing a tipping hazard. In the middle of Room #1, at the bottom of the two shelf, wooden bookcase's veneer is lifting posing a splintering or cutting hazard. In the Preschool Room a folding bookcase on the wall across from Room #2 had missing veneer and a chipped corner posing a cutting and/or splintering hazard. In the Preschool Room, a 2 x 3 shelf next to the glass exterior door was broken posing a splintering hazard. In the Preschool Room, loose screws were observed on a blue, art, drying rack to be unstable posing a fall hazard.
The facility's rest equipment was ripped that exposed the foam padding.
The Emergency Contact Forms for Child Files #1, 2 and 3 lacked the "Enrolling Parent's Work Address and Work Number." (SEE LIS CODE SHEET)
The written emergency transportation plan was lacking from Rooms #1, #2, #3 and #4.
