In Room #3, a bin with plastic animals was cracked posing a cutting hazard. In Room #4, the bigger room, on the right wall, under the TV is an "L" shaped hook which is loose with exposed edges and screw posed a puncture and/or cutting hazard.
The Initial Health Assessment for Staff # 2 was dated after their hire date.
In an unlocked, child-height counter drawer, in Room #1, push pins, a pair of adult scissors and screws which pose a cutting and/or puncture hazard were accessible to children.
In Room #5, next to the sink on the floor, was a child rocking chair seat with exposed nails which poses a cutting and/or puncture hazard.
