During an unannounced inspection on 6/5/2025, peeling paint was observed on the wall in the front area of the room located on the left side of the building upon entering the facility.
On 6/5/25, Staff #1 had a volunteer Pennsylvania child abuse clearance on file, which is not suitable for employment purposes. Staff #1 was observed to have direct contact with children during the inspection. See code sheet for date of hire.
This change in location capacity inspection recorded no violations or advisories.
On 10/23/2024, Staff #1 it was discovered that Staff #1 has resided outside of the state of Pennsylvania during the previous 5 years. Staff #1 did not apply for the out of state clearances prior to employment as required by the CPSL. Staff #2 had current clearances on file on 10/29/2024. However, the updated state police clearance on file for Staff #2 was dated 8/6/2024. The previous state police clearance on file for Staff #2 was dated 4/23/2019.
During the Renewal Inspection on 10/23/2024 and on 11/5/2024, the facility did not have a qualified director present at the facility site for a minimum of 30 hours per week.
A renewal inspection occurred at the facility. A review of children's files was conducted. See the code sheet for the date of enrollment for the children. Regulation requires children's health reports to be obtained no later than 60 days following the first date of attendance at the facility. The following children's health reports were not obtained within 60 days: Child # 1 and Child # 2. Child # 1 obtained a health report on 12/26/23. Child # 2 obtained a health report on 1/2/24.
A renewal inspection occurred at the facility. A review of staff files was conducted. See the code sheet for the date of hire. The file of Staff # 1 indicated this staff had lived out of state within the past 60 months. As a provisional hire, Staff # 1 was not hired correctly as a provisional hire as per the CPSL because they did not apply for the out of state clearances before working in child care. Staff # 1 was working at the time of this inspection. Staff # 2 was hired as a provisional hire but did not apply/receive for the FBI clearance until 12/18/23. The file also did not contain an NSOR clearance. Staff # 2 was hired incorrectly as a provisional hire because they did not apply for the FBI clearance at the time of hire and the file did not contain an NSOR clearance prior to the ending of the 45-day provisional hire period.
