During the renewal inspection on 10/15/2025, three trash cans in the facility without lids were observed with discarded food and other contaminated items.
During the renewal inspection on 10/15/2025, Staff #1 was observed changing diapers for five different toddler children. It was observed that Staff #1 did not wash the child's hands or their own hands after changing the diapers. In addition, prior to lunch it was observed that neither Staff #1 nor Staff #3 or any of the toddlers in the group washed their hands.

During the renewal inspection on 10/15/2025, the rest cots used by the preschool and toddler children and the pack and plays used by the toddler children were not labeled for the use of a specific child to be used only by the specified child. In addition, the pack and plays in the infant area did not have at least 2 feet of space on three sides while they were in use.
During the renewal inspection on 10/15/2025, the vinyl on the play chairs in the preschool area was torn and had foam exposed.

It was verified through staff interviews that on the morning of 7/8/2025, there were maggots observed inside the facility. Staff reported that this was caused by trash from the outside being brought inside overnight rather than taken to the dumpster due a rainstorm at the close of business on 7/7/2025, and a bear in the neighborhood that was getting into the trash.
During an unannounced inspection on 7/10/2025, it was discovered that Staff #1 has not completed mandated reporter training within the last 60 months. This is evidenced by the previously documented mandated reporter training on file being dated 3/17/2020.
During the Renewal Inspection on 10/8/2024, it was discovered that one of the two written references on file for Staff #1 was verified to be from a family member.
The facility self-reported that on 7/22/24 at 1:16 p.m., Staff #1 was observed on swatting the head of Child #1, age 2, twice with an open hand when Child #1 bit the leg of Staff #1. This incident was confirmed on 8/1/2024 when the video was reviewed by the Certification Representative.

An unannounced inspection occurred at the facility on 11/17/21. A review of staff files was conducted. Staff # 1 had a date of hire of 8/3/21 and Staff # 2 had a date of hire of 10/4/21. Both staff were working with children at the time of this inspection. Both staff files were missing proof of education and could not be appropriately qualified as a staff person.
An unannounced inspection occurred at the facility on 11/17/21. A review of staff files was conducted. Staff # 1 had a date of hire of 8/3/21. At the time of this inspection, Staff # 1 did not have documentation of completion of the health and safety topics. This training is required to be taken within 90 days of a staff's hire date.

The CPSL requires an approved provisional hire waiver to be able to hire staff on a provisional basis. At the time of this inspection, this facility did not have an approved provisional hire waiver and the director was attempting to hire Staff # 1 and Staff # 2 as provisional hires. Staff # 1 was working with children and the file did not contain a state police, child abuse, FBI or NSOR clearance. Staff # 2 was working with children and the file did not contain an FBI or NSOR clearance.
