At time of unannounced inspection on 1/29/26, chipped white paint was observed by the changing table in the young 2's room, and chipped yellow paint was observed on the corner of the wall by the emergency exit door in the young toddler room.

At time of complaint investigation on 12/23/25, it was observed that staff person #4 and #5 did not ensure that children's hands were washed prior to eating lunch. Children were observed going from the carpet, to get lunch boxes, and then to the lunch table to begin eating without hands being washed.
At time of complaint investigation on 12/23/25, staff person #2 acknowledged that staff person #3 started a verbal altercation with staff person #2 on 12/5/25, which caused supervision to be compromised. During the altercation, child #1 fell and was later diagnosed with a concussion.
At time of complaint investigation on 12/23/25, it was observed that staff persons #4 and #5 did not wash their hands prior to helping children with their lunches.
At time of complaint investigation on 12/23/25, staff person #1 acknowledged that child #1 received care from a medical professional on 12/6/25, for an injury that occurred at the facility on 12/5/25. The Regional Office was not notified by the facility within 24 hours of the facility finding out the child was treated, and a written report was not submitted by the facility within 72 hours, as required.
At time of complaint investigation on 12/23/25, staff person #2 acknowledged that staff person #2 and #3 were involved in a verbal altercation in the classroom on 12/5/25, and loud, harsh language, heard by the neighboring classroom, was used in front of the children.
At time of unannounced inspection on 12/23/25, an infant was observed sleeping in a crib and there was not 2 feet of space on three sides of the crib as required.
At time of renewal inspection on 10/28/25, a set of bottles for one of the infants was observed in the refrigerator and the bottles were not labeled with the child's name.

At time of renewal inspection on 10/28/25, several ceiling vents throughout the facility were observed with visible dust and dirt. Peeling white drywall/paint was observed at the bottom of the wall by the computer in the Jr. Pre-K room.
At time of renewal inspection on 10/28/25, the health assessment for staff person #1 was more than 24 months old, with a date of 8/2/23.
At time of renewal inspection on 11/19/24, the hot water temperature in the young toddler room measured at 119 degrees Fahrenheit, using a thermometer.
At time of renewal inspection on 11/19/24, expired Albuterol medication was observed in the cabinet in the Pre-PS room.

During unannounced inspection 7/31/24, hand sanitizer was observed on the shelf in the Jr. Pre-K room and was accessible to children.
During unannounced inspection on 7/31/24, two outlets were observed in the Jr. Pre-K room that were not in used and did not have protective covers in them., and were accessible to children. Also observed was a phone jack dangling from the wall. The phone jack was not in use and was accessible to children 5 years of age or younger.
During unannounced inspection on 7/31/24, the following physical site items were observed at the facility: the ceiling vents in several classrooms were visibly dirty and in need of being cleaned. In the infant room, the covering on the side of the refrigerator was peeling and was accessible to the children. In the Pre-K 1 room the drywall or surface around the bottom of the widow was observed to be crumbling onto the windowsill.
During unannounced inspection on7/31/24, chipped paint was observed in the school age room by the closet and under the windowsill, in the Jr. Pre-K room under the bulletin board, and in the Pre-K 1 room by the windowsill.
During walkthrough at renewal inspection, ceiling intake vents throughout the facility were observed to be visibly dusty and dirty. Windowsills in the Pre-K 2 room were observed to be visibly dirty and one of the windows had a spot of rust. Several ceiling tiles in the infant room were observed to be stained.
During paperwork review at renewal inspection, the facility's emergency plan had not been updated to meet Federal requirements under 45 CFR § 98.41(a)(1)(vii), which includes having a plan for Continuity of Operations
During file review at renewal inspection, staff persons #1 and #2 had not completed pediatric first aid and CPR from an approved curriculum, or trainer within 90 days of employment.
This renewal inspection recorded no violations or advisories.
Certification Representative observed children in the Pre-school classroom were not being properly supervised as required during naptime due to the two staff in the room were on a computer with their backs turned to the children.

At time of renewal inspection on 10/28/25, the health assessment on file for staff person #2 did not include the physician's answers to the questions regarding communicable diseases and the individual's suitability to provide child care.
During walkthrough at renewal inspection, chipped white paint was observed on the windowsill in the Pre-Tod room and near the closet and coat racks in the Pre-K 1 room, as well as on the wall behind the shelf in the Pre-K 2 room.
During walkthrough at renewal inspection, rusty stall dividers were observed in the boys bathroom and rusty water line covers were observed on the walls in the boys and girls bathroom.
