At approximately 10:00 a.m. CR observed 14 preschool aged children in the preschool room with staff #1 and #2. Staff #2 went into the infant room adjacent to the preschool room to tell the designated staff in charge CR was there. By doing this, staff #2 left staff #3 out of ratio with 14 preschool age children.
On 4/24/24 the first aid kit in Room D lacked soap and disposable, nonporous gloves.
On 4/24/24 the sink in Room B did not have platforms or steps to be easily approached.
At approximately 2:00 PM on 4/24/24 Certification Staff observed 1 preschooler lying on rest equipment behind a bookcase in Room C which did not allow for critical oversight in which the supervisor could see, hear, direct, and assess the activity of the supervisees.

The director indicated child records were not stored in a locked cabinet.
