There is not a plastic-lined, hands-free covered can for disposing soiled diapers in the young toddler room.
Staff Person #3 does not have a complete PA State Police clearance on file, as the status is listed as under review. Also the DHS FBI clearance is not complete.
A child's medical information was posted in the young toddler room where was visible to anyone who walked into the room.
Staff person #1 has completed Pediatric first aid and CPR training, however the training was not completed through a Pennsylvania Quality Assurance System (PQAS) approved curriculum. Staff person #2 has not completed the following required pre- service training within 90 days of their date of hire: pediatric first aid and CPR (see LIS code sheet for date of hire) Staff Person #4 completed PA Required Health and Safety for Centers-Group Child Care Part 1 AND PA Required Health and Safety for Centers-Group Child Care Part 2 on May 20, 2025 which was not within 90 days of hire. In the infant and young toddler rooms, diaper rash creams and petroleum jelly were accessible to children in the changing tables.
In Fellowship Hall there were heating grates on the walls which were not flat against the wall leaving areas where a child could get their finger stuck. In the preschool room, an area rug had the corners turned up posing a potential tripping hazard.
The refrigerator in the infant room did not have a thermometer. There was also something spilled that had not been cleaned up.
Staff Person #1's initial health assessment was conducted 4/18/23 which was more than 12 months prior to her start date. See code sheet for first day caring for children.
It was observed in room 1 that two-year-old children had access to and were playing with play dough that specified to be only for children 3 and up. The play dough was removed from the classroom during inspection.
The parent letter did not contain all of the required parts. The parent letter was missing a lockdown section and accommodations for infants and toddlers. It was observed that infants and toddlers were in care.
Staff #1 has lived out of state within the last 5 years. Staff#1 had all PA state clearances, however did not also have out of state clearances as well.
At the time of inspection, staff person #1 and #3 were the only two staff persons employed at the facility that would be caring for children. Neither staff person #1 nor #3 have completed the following pre-service training required prior to caring for children unsupervised: although staff person #1 has completed the original 10 health and safety training topics, they have not completed the one hour revised health and safety training, and staff person #3 has no verification of completing all 10 health and safety training topics.
It was observed that the first aid kits in room 3 and 4 were stored in an area that allows for them be accessible to children.
It was observed in rooms 1-4 that the string from the window blinds hung low enough to create a possible choking hazard for the children in care.
There was an uncovered plate of food with a fork in the refrigerator within the facility.
The facility did not have child care centers regulations accessible or posted in a conspicuous location for the parents.
The facility did not have documentation of notifying safety authorities in writing of the location of the facility and the pedestrian and vehicular routes around the facility.
The facility did not have their emergency plan posted in a conspicuous location.
Chipped and peeling paint was observed on the door in the Fellowship Hall.
It was observed that the only fenced outdoor play space that will be used by all age groups at the facility, infant to school age, contained rubber mulch. There is nothing present to keep the mulch inaccessible from the infants or toddlers that will be in care in that space.
The file for staff person #2, did not contain adequate verification of their education level and did not contain two years of verified experience working with children. Staff person #2 was hired to be an assistant group supervisor.
File for staff person #3 did not contain two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person. Only one was observed on file at the time of inspection.
File for staff person #1, who is employed as the facility director, did not contain the NSOR verification certificate. File for staff person #3, who is employed as the only second required staff person, did not contain documentation of applying for or obtaining a state police clearance or the NSOR verification certificate.
