On 3-10-2025, a fraudulent certificate of completion of the Health and Safety training (Part 1 and Part 2) was submitted to the regional office for staff person #2. Trainings were dated 4-4-2023 for Part 1 and 4-6-2023 for Part 2. Staff person #2, when interviewed, acknowledged that they did not take the Health and Safety training in 2023.
