The center failed to make a report to the licensing authority within 24 hours after the incident occurred regarding an accident that required medical care beyond on-site first aid. The temperature in rooms used by children is less than 68 degrees Fahrenheit. In schoolage classroom reads 64 degrees. Immediate - Director has contacted the center grounds keeper who will be onsite 12/12/2025 same day to turn on heat/install thermo window covers. Preventative - Eff 12/12/2025 Director will conduct periodic checks throughout the school day to ensure temperature is within required range (68 to 82 degrees). The child(ren) in the Preschool class room(s) was/were left unattended when one educator went inside leaving eight children outside at the playground for approximately one to two minutes. . Immediate Action: The provider will maintain a structured teaching schedule with appropriate schedules and ensure that there are always enough staff present. The provider will ask educators to give a 24 hours' notice of any days off requested. Teachers will add to classroom rules how children should stay together for their safety and to always ensure visibility and review with children daily as they transition throughout the day. i.e. Circle Time, Before Outdoor Play Transition, etc. Preventative Action: March: All teachers were informed of the change in attendance. The topics discussed were free time and late arrivals. Both are important to maintain a routine not only for the children, but to maintain a safe and professional environment for all teachers and children and respect for each other's time in the classroom.
