Repair the electrical outlet on the second floor in room 10 which had caught fire. The outlet was burned, melted, and there were flame marks up the wall above the outlet. The center advised they received an email from the cleaning crew that the outlet fire happened the night before on 3-16-26. OOL called the local fire official, the fire department responded, and completed a safety check of the building. The breaker to the outlet was shut off and the center was advised they needed to make the necessary repairs by 3-31-26.
Cease the use of the Little Tikes slide and blue horse Teeter Totter in room 2.

Ensure that children are directly supervised at all times. In room 1, infants on the floor for tummy time need to be directly supervised at all times. In room 2, a child was standing up on top of a little tikes slide without staff intervening. In room 4, children were walking with toys in their mouth without staff intervening. In room 1A, a child was climbing up on the stroller and playing with the buckles. 4-14-26: In room 2, children were climbing on top of tables and playing with high chair buckles without staff intervening.
Ensure children do not have access to the ripped and peeling blue sky paper, on the glass wall, between rooms 1 and 1A.
Ensure that parents are notified immediately when: a child is bitten and the skin is broken; a child sustains a head or facial injury, including when a child bumps his or her head; a child falls from a height greater than the height of the child; or an injury requiring professional medical care occurs. Provide a current health certificate as required.
Ensure children do not have access to blind cords or sound machine cords throughout the center. Ensure that children do not have access to the cell phone charging cord in room 7.

Room 2A and all of the classrooms on the 2nd floor need the required amount of supplies. 2-22-24: The center removed the second floor from their license and room 4 is now for occasional use.
Ensure that adequate supervision is maintained at all times throughout the center. Ensure older children in room 1 are not holding infants and talking on cell phones.

Provide bottle warmers where needed. 11-13-23: The center was using hot water from a water cooler to make bottles and storing it in the classrooms in a thermos. The water temperature measured at 171 degrees. The center needs to cease the use of hot water to make bottles.
Develop a written policy for parental notification of injuries if the center uses methods other than a telephone call and maintain on file the signatures of staff and parents acknowledging receipt of the policy. The center indicating they are sending text messages also.
Ensure that at least two staff have current first aid and CPR certifications and are present at all times at the center.
Ensure that all Epi-pens and required medications are onsite at the center, stored in their original boxes with the child's name and prescription, and accompanied by a special care plan from a health care provider. Ensure that all staff who do not have a completed CARI and CHRI background checks and orientation training are directly supervised by another staff member. None of the center staff in classrooms with children had the required CARI clearances.
