Ensure that children are directly supervised at all times including during naptime. In multiple classrooms, the only staff in the classroom during naptime was sitting down with their heads down entering information into center's communication app. Additionally a few staff also were unable to maintain direct line of sight with the sleeping children as the children were positioned behind shelves.

Ensure that all cribs, cots and sleeping equipment are arranged to provide access to a three foot wide aisle that leads to an exit. On the day of the inspection numerous classrooms were blocking egresses during nap time. All classrooms addressed blocked egress by OOL.
Ensure that the staff utilize positive methods of guidance and discipline at all times throughout the center. A named staff in room 106 was observed lifting and guiding the children by one arm instead of using two hands on the torso. Named staff person was addressed by OOL at the time of the inspection.
Ensure that the staff in room 106 utilize age and developmentally appropriate methods of guidance at all times throughout the center. A named staff was observed using harsh words and language in the classroom and when responding children not listening. Staff person was heard yelling "NO" to children repeatedly without any further guidance or positive redirection.
Retrain the named staff on the center's policies and procedures for safely lifting and guiding children throughout the classroom using a two hand on the torso method. Provide training documentation with the staff's signature attesting to the training completion.
Ensure that materials are accessible to the children at all times in room 104, except during lunch and nap time.
Ensure that the staff utilize positive methods of guidance at all times throughout the center. A named staff was observed lifting and guiding the children by one arm instead of using two hands on the torso.
Ensure that the fire extinguisher in the bathroom of room 118 is serviced and tagged annually and recharged as needed. Additionally ensure that this fire extinguisher is secured properly to the wall.
Ensure that the center's fire protective systems are operative at all times, whereas in room 115, the fire monitoring system in the ceiling was covered with plastic and tape.
