Replace the rusted and unsecure handrail on the exterior steps leading from the play area to the side stairwell.

Ensure that staff encourage children to participate in activities and that the children are provided a mixture of active and quiet experiences. The 10 children in the school aged summer group was not provided age appropriate activities after lunch and were on their personal devices for multiple hours.
Ensure that the use of television, computers, and other video equipment is for educational and instructional use, is age and developmentally appropriate, is not used as a substitute for planned activities, and is not used for passive viewing. School aged children were observed watching non-educational YouTube videos on their personal devices for extended periods of time throughout the day.
Ensure that pacifiers are removed when children are crawling or walking in Room 1.
Provide age appropriate materials and furniture for children ages 5 and older throughout the center. Multiple children in the school aged group were observed laying on the floor and using preschool sized chairs since no age appropriate furniture was available at the time.
Ensure that the center completes and maintains on file a completed children's records checklist.
Ensure that the center washes and disinfects surfaces immediately as required, whereas a named child's diaper fell off and leaked onto the floor and the staff only used paper towels to dry the mess without using a two-step process to clean and disinfect the area in Room 1.
Ensure blankets, bibs and pacifiers are not draped over the side of the crib when in use by children in Room 1.
Ensure that food provided by the center is stored, prepared and served in a sanitary manner. The milk and frozen food products delivered by the food program were left out in Room 1 for multiple hours before being stored properly in the refrigerator.

The following items were observed in the second floor kitchenette: 1) a bowl of rotting fruit with fruit flies present, 2) open and dirty condiments on top of the refrigerator, 3) open packages of food inside the refrigerator, freezer and on the counter, 4) drawers of food sitting in water inside the refrigerator, 5) a bin of dry good sitting in standing water on the floor under the sink, 6) staff food, including take out containers, mixed with the food supplied by the Food Program.
Cease the use of pacifier straps. There were 3 pacifiers with straps attached in the feeding chairs in Room 1.
Ensure the health, safety, and well-being of the children served, whereas staff placed multiple children, 3-18 months, at the feeding tables without fastening their safety straps. 11/28/23: There were 2 children, under 18 months, sitting in the feeding table without a safety strap provided and one child, under 18 months, with an unbuckled safety strap at the feeding table in Room 1.
Repair the wooden fence gate that is not latching properly in the outdoor play area.

Hire a director and submit the required documentation for education and training experience, as the center has not had a director for more than 6 weeks.
Ensure that adequate supervision is maintained at all times throughout the center, whereas a 3 year old child was sent upstairs without staff supervision. A second staff coming down the stairs was unaware the child was on the landing behind the closed door and opened the door accidentally knocking into the child.
Repair and paint the hole in the wall on the left of the doorway in Room 4.
Ensure the health, safety, and well-being of the children served, whereas staff placed 5 children, 4-18 months, at the feeding tables without fastening their safety straps. One of the 5 children attempted to climb out of the seat onto the table.

Remove the bottle warmer on the mini fridge in Room 1 and ensure it is turned off when not in use as it was observed boiling with excessive steam.
Eliminate moisture resulting from the steam radiator leak in the second floor kitchenette.
Ensure that walking trips do not involve entrance into a facility unless the facility has been indicated on the blanket permission slip. The center walked 28 children, ages 2.5-13 years old, on a field trip to Milo S. Borden Park without the proper permission slip signed by the parents. Additionally, the center takes children to the same park daily without written consent from the parents.
Ensure that the center complies with the licensed capacity of the classrooms at all times throughout the center, whereas: 1) There were 10 children present in room 2, exceeding the room capacity of 9. 2)There were 12 students present in room 5, exceeding the room capacity of 9.
Submit a Safe Building Interior Certification or other approval issued by the DOH.
Ensure that infants are provided age-appropriate, supervised tummy time at least twice per day, whereas an infant, below 12 months old, was moved between the bouncy seat, crib and being held and not provided with tummy time or floor time throughout the day.
Eliminate moisture from the steam radiator leak in the second floor kitchenette that has resulted in an active leaking ceiling in Room 1 Infants.
Ensure that all outdoor hazards are removed as required. Secure or remove the outdoor turf as it is loose, bunched and poses a tripping hazard.

Staff placed dirty pacifiers in two named infants mouths without washing and disinfecting the pacifiers after they had fallen on the floor and soiled diaper changing station.

Additionally, one named staff in Room 1 was observed taking children to the bathroom and supervising a group of sleeping children at nap time without an additional staff present with the required background clearances on file.
Ensure that the ABC mat/carpet is securely fastened to the floor in Room 1.
Ensure that all Epi-pens are stored in their original boxes with the child's name and prescription, accompanied by a special care plan and that two are present when required in room 5.

Ensure that time frames for each activity are both age and developmentally appropriate for the children served. Children, ages 0-18 months, were sitting and waiting at the tables for over 20 minutes with no activities provided.
Ensure that both staff-directed and child-selected activities are provided in Room 1, while outside on the play area and during transition times. Children were not provided activities, except free play, in Room 1 throughout the day.
Ensure that adequate supervision is maintained at all times throughout the center. Staff was unaware and did not intervene when a named 3 year old, from room 1, picked up a bottle cap from the play area and put it in their mouth.
Ensure that adequate supervision is maintained at all times throughout the center, staff from Room 1 were unaware that children, ages 24 months and under, were playing with exposed cut fencing pipes and loose/missing fencing panels while on the outdoor play area. 7/31/23: Staff were unaware that 2 named children, 24 months and under, were playing with exposed cut fencing pipes while on the outdoor play area.
Ensure the health, safety, and well-being of the children served. Staff were unaware that screws were on the floor and windowsill, and a backpack was wedged between the radiator and wall in Room 4. Staff took corrective action to remove the items at the time of the inspection.
Ensure that children wash their hands immediately after coming into contact with blood, nasal secretions or other bodily secretions in room 5.
Ensure that a primary caregiver is assigned to each group of four infants or six toddlers in room 1.
Ensure that all exits and egress areas are unobstructed and that the emergency exit doors are easily operable. Multiple cases of bottled water and materials were on the egress stairs from the second floor and blocking the front exit doors from Room 1.
Submit a lead paint risk assessment as required.
Ensure the health, safety, and well-being of the children served, whereas management has allowed classrooms to be occupied by children, ages 0-6 years, when hazards were present such as no heat on the second floor multiple times, and active water leaks into the infant room. Provide the OOL with a signed and dated MOU indicating that management will not put children at risk of harm when facility issues arises and will take the necessary steps to ensure the safety of the children, including but not limited to moving children to another classroom if space allows and/or closing the classrooms/center when needed.

Ensure that pacifiers are removed when children are crawling or walking.
Ensure that all cribs and playpens are equipped with all of the required components, including a firm, waterproof, snugly fitting mattress.
Ensure that unapproved space is inaccessible to the children at all times, whereas the following occurred: 1) A named child was brought into the unapproved basement to heat up food with a named staff. 2) The unapproved basement has been set up with tables and cubbies for the school age summer program.
Ensure that age appropriate tables and chairs for the infants and toddlers in Room 1 who no longer need to be held and have out grown the feeding table bucket seats.
Ensure that each child's bottle(s) are labeled with the child's name and dated in Room 1.
Provide the CHRI clearances for 2 named staff members.
Submit the director's qualification requirements for education and experience.
