A named child grabbed another named child's pacifier with a attachment and was pulling it to put it in his mouth and staff were unaware in Room 1. b. Both staff in Room 1 was in the kitchen area with their back turned while 6 children, ages 0-18 months were on the floor playing unsupervised. c. During transitioning from the playground 6 children were in the Room 4 and the bathroom unsupervised while the staff were outside trying to get the children in the room.
