Ensure that all staff complete orientation training within two weeks of hire for two staff.
Ensure that at least two staff members who have current pediatric first aid and CPR certifications are present at the center at all times and that the training includes a hands-on component in addition to any online training.
Ensure fire drills are conducted during each session provided at the center and all children are evacuated from the building within three minutes during each fire drill. Only 1 fire drill has been documented each month (AM and PM drills are required monthly).
Ensure that CARI background checks are completed as required for the sponsor/sponsor representative and all regularly scheduled staff. Program is missing 2 CARIs.
Complete and maintain daily time sheets with arrival and departure times for staff.
Ensure that children are adequately tracked at all times and staff are aware of the number of children in their care. When asked, the staff member was unaware of the number of children in her care. 11.25.25- Staff stated 8 children were present, wherein 7 children were actually present. 12.15.25- Staff stated 22 children were present with 5 of them being under 5 years old, wherein 32 children were present with 4 of them being under 5 years old.
Limit group size to 30 school-age children. There were 34 school aged children and 13 preschoolers in one large area. Divide groups by putting the preschoolers in another classroom and separate school aged children into smaller groups.
Ensure that the center cooperates with all DCF inspections as required. At the time of the inspection, the director was uncooperative and was unwilling to answer questions regarding outstanding violations from the previous inspection.
Ensure Epi-pens are stored in their original boxes with the child's name and prescription, accompanied by a special care plan that includes procedures for contacting parents and emergency medical services, and both epi-pens are present when two are required. One child is missing asthma medication and a second epi-pen, per their Special Care Plan. Another child is missing their Special Care Plan. 12.15.25-One child is missing her Asthma medication and second epi pen, as needed per her Special Care Plan.
Ensure that adequate staff/child ratios are maintained at all times. When preschoolers were moved to their classroom, there were 34 children with 2 staff when 3 are required.
Ensure that children are directly supervised at all times including walking to and from the bathroom. Two children were observed in the hallway, unsupervised, walking back from the bathroom. 11.25.25- A staff member left children unattended multiple times, in the gym, to answer the front door, while the second staff member was in the cafeteria.
