Ensure that all outdoor equipment is sturdy, safe, free of hazards and used in accordance with the manufacturer's instructions. Repair and/or remove all rusted equipment from the outdoor play area.
Ensure that the center has an adequate supply of materials to meet the daily activity needs of the children and the program and ensure all materials used by the children are age-appropriate, non-toxic, and safe for use by children in room #4.

At the time of the inspection, the center director disclosed that an incident in May occurred involving a child being unaccounted for during their return from their daily outdoor play time. The center was directed to call the hotline and report the incident.

Ensure all surfaces are in good repair including behind the toilet. At the time of the investigation rust on the plumbing equipment was observed in the hallway bathroom.
