DOH reported that, "asbestos floor tile is located throughout your childcare center and was in good condition during the DOH site visit inspection. Also, the pipe insulation in the boiler room is a mix of fiberglass and presumed asbestos pipe joint insulation –all in good condition at the time of the DOH site inspection. Lastly, there remains presumed asbestos pipe insulation on the pipes above drop ceiling tiles in your childcare center—no damage identified during the DOH site inspection. Please continue to ensure these asbestos and presumed asbestos-containing materials remain in condition. If any of these materials need to be removed or repaired in the future, this must be conducted only by a NJ Dept. of Labor licensed asbestos abatement contractor. Center must submit a MOU to OOL regarding the asbestos action plan.
Ensure that CARI background checks are completed as required for the sponsor/sponsor representative and all regularly scheduled staff for the center's license renewal.

Ensure that a Universal Health Record is completed upon enrollment and updated annually, including the immunization records and special care plans as required.
Ensure children sign-in attendance sheets are maintained daily.
Repair the boys bathroom dividers.
Ensure exit door is easily operable in classroom 2.
