Submit a copy of the final Certificate of Occupancy that indicates the correct use group for the children served.

Based on information, ensure that adequate procedures for tracking children are adhered to at all times throughout the center. A named staff failed to notice that a named child had left the center unsupervised. Additionally, staff had indicated that tracking is inconsistent at the center. On 11/30/2021, staff were unaware of the number of children in their care.
Provide documentation that the center (supplied by a community water system): completed water testing conducted by a Department of Environmental Protection (DEP) certified lab for water testing for lead and copper from all faucets and sources used for drinking water or food preparation and at least 50% of all indoor water faucets used by the center. Submit DCF Drinking Water Checklist and Testing Statement of Assurance.
Per the Updated COVID-19 Standards – Child Care Centers, ensure that groupings are separated by no less than 6 feet in all directions from other groups or separated by walls or other physical partitions. Prohibit interactions between groups and limit comingling of groups to periods when total attendance is substantially decreased (e.g. the last hour of operation), when comingling is limited to sibling groups or social distancing of at least six feet is strictly observed, and mask use for children over age 2 is required, subject to medical limitations.

Staff were advised that the exits could not be obstructed and the obstructing furniture was moved immediately. On 6/17/2021, The same staff member placed furniture in front of the emergency exit to ensure that a named child did not exit. Provide a corrective action plan to the Office of Licensing for ensuring that emergency exits and egresses remain unobstructed while caring for children with special needs known for wandering or elopement.
