At the time of the inspection, the director designee informed me of an emergency exit door that was inoperable due to a fire they had months ago. Firefighters had to break in through that door and had since been left jammed shut. There are more than 2 other emergency exits that are operable in the center.

Upon entering the room, the teacher did put the child on their back. When asked if the child could roll over, the director said no.

Ensure that the staff/child ratios are maintained at all times, wherein based on complaint #19119 and through observations: a) at 9am 8 children (4 children, under 18 months old, 3 children, 1.5-2 years old and 1 child 2.5 years old) were present with 1 staff in Room 1 and an addition staff was required; b) at 10am 11 children (7 children, under 18 months old and 4 children, 1.5-2 years old) were present with 2 staff in Room 1 and an addition staff was required; c) at 12pm 12 children (7 children, under 18 months old and 5 children, 1.5-2 years old) were present with 2 staff in Room 1 and an addition staff was required; d) at 3pm 12 children (7 children, under 18 months old and 5 children, 1.5-2 years old) were present with 1 staff in Room 1 and 2 addition staff members were required; e) at 9am 16 children (2.5-5 years old) were present with 1 staff in Room 2 and an additional staff was required; and, f) at 1pm 29 children (13-3 years old, 8-4 years old and 8- school-agers) were present with 2 staff in Room 2 and an additional staff was required. Thirty minutes after the OOL inspector was present, staff from the main office arrived and called additional staff from their other locations to cover classrooms. It was observed that when the OOL inspector left, the staff from the other locations left also. When the OOL inspector returned at 11:45 staff/child ratio were not maintained.

Limit the use of infant equipment including bouncy seats to periods of 30 minutes at a time, wherein 8-12 children, ages 0-2 years, were placed in either a chair at a table, a low highchair or bouncy seat and were buckled in upon arrival, through lunch and until the children were changed and put in a crib for nap. Four of those children continued to stay in the bouncy seats for naptime. A named staff stated she needed to keep them buckled in the seats to keep them safe because they were short staffed and she could not adequately supervise the children if they were free to crawl and walk about the room.
