Based on information, ensure that adequate supervision is maintained at all times throughout the center. Three named staff were unaware of how a named child sustained superficial scratches to their face.
Ensure that all toxic substances are inaccessible to the children. At the time of the inspection, there was a hallway closet that was unlocked with cleaning supplies.

Ensure that bottles are not propped at any time in Room 1. At the time of the inspection, an infant was drinking from a propped bottle in a rocker seat in Room 1.

At the time of the inspection, 2 staff members were asked how many children were present in their care and both responded incorrectly. 1 staff member said there were 24 children present and the other said 29 children were present. There were 25 children present.

Ensure that the center's fire protective systems are operative at all times. At the time of inspection, the fire panel was in trouble mode. Fire company restored the fire protective system to normal status.
Ensure that the area rugs throughout the center are kept clean.
Ensure that each Type II School Bus is equipped with "S2" plates and meets all applicable provisions as specified in the manual. As per MOU, transportation is no longer provided.

Based on Complaint #19136 and as per the NJ Department of Health COVID-19 Public Health Recommendations pursuant to Executive Order No. 149, ensure that the center immediately separate a child or staff member that has had contact with a positive case, wherein the class/center was not notified to quarantine after an exposure until 24 and 48 hours after. Positive case Wednesday morning, blue class notified Thursday afternoon, entire school notified Friday and quarantined for 7 days.
Ensure that staff/child ratios are maintained at all times, wherein there were 13 children 1.5-2 years old present with 2 staff in Room 2 and an additional staff was needed.
Ensure that children are supervised at all times wherein, a named 3 year old child from Room 4 walked outside the classroom, around the corner and into the bathroom unsupervised by a staff.

Ensure that the toy shelves are not overloaded, wherein toys were stacked on top in Room 1.

Ensure that all staff are aware of the number of children in their care at all times, wherein a named staff member stated there were 18 children in room 1 and 19 were present.

Based on information, ensure that the staff utilize positive methods of guidance and discipline. A named staff pushed a named child's forehead backwards as a means of discipline. The named staff also admittedly feigned pouncing on students when misbehaved.

Based on Complaint #19314 and through observation: ensure that the tables are washed and disinfected before each meal, wherein a named staff member only used one step process to wash and disinfect the tables in Room 2. Additionally, the named staff used the same disposable paper towel on all tables and highchair trays.
Ensure that staff set aside toys that children have placed in their mouths, wherein a named staff put a yellow lego mouthed by a child back in the bin of legos in Room 3.

Ensure that a crib is provided when a child is asleep, wherein 4 infants were sleeping in bouncy seats and 1 infant was sleeping in the bucket seat table in Room 1.
Ensure that ratios are maintained during naptime, wherein; a. sixteen children, 18-24 months were napping with 1 staff present and an additional staff was needed, and b. 21 children, 2.5-3 years were napping with 1 staff present and an additional staff was needed.
Ensure that the center maintains the licensed capacity in each room , wherein Room 5 is licensed for 21 and 23 children were present during naptime.
Ensure that the center meets staff/child ratios at all times, wherein a) 19 children, under 18 months old were present with 3 staff and 2 additional staff were required in Room 1; b) 13 children, 1.5-2 years old were present with 2 staff and 1 additional staff was required in Room 2.
Ensure that the staff knows the number of children in their care at all times, wherein a named staff stated there were 12 children present and then counted and said there were 13 in Room 2. 2/11/2020: A name staff stated there were 13 children in their care and there were 12 children present in Room 2.
