This Statement of Findings includes a repeat citation and failure to comply with an approved Corrective Action Plan for He-C 4002.41(a) Corrective action: Staff completed the NHCIS form on 9/9/24. In the future, staff will not be allowed on the property until deemed eligible by CCLU.
REPEAT CITATION PREVIOUSLY CITED ON FEBRUARY 2, 2022. The licensing coordinator reviewed program records and determined that one staff did not complete the background record check application through NHCIS prior to employment or residency and the licensing coordinator observed the staff working in the program before being deemed eligible by the Unit. This constitutes a repeat citation and failure to comply with one approved Corrective Action Plan. Corrective action: staff completed the NHCIS form. In the future i will double check to make sure all forms are complete and staff will not be allowed on the property until deemed eligible by CCLU.
