The program shall maintain daily child attendance records, in either paper or electronic format, including the child’s full name, date of birth, and their arrival and departure times in the program and each room or group throughout the day, as applicable, recorded in real time by staff or parents.

A statement by the complainant indicated that required staffing ratios were not being met. The licensing coordinator's review of staff and child attendance records from June 1, 2024 through July 23, 2024, revealed that the staff attendance was incomplete, as program staff were not signing in or out in real time as required. Corrective action: Staff will need to make sure they are clocked into the correct room they are working in. When we need to switch rooms staff need to switch in live time to keep attendance records current. I will double-check in the Procare app to make sure everyone is in the proper room.

A review of the program's attendance records revealed that they were incomplete and could not substantiate whether appropriate staffing ratios are being maintained. Two staff members stated that required ratios are not being maintained, and one staff member stated that it has been difficult to maintain appropriate ratios after a former staff member left employment at the program. Corrective action: Staff will need to make sure they are clocked into the correct room they are working in. Blankets, sleeping bags, bedding, cots, and mats shall be stored in a manner that ensures that sleeping surfaces are not touching, or the items shall be washed and sanitized before re-use if stored in a way that sleeping surfaces are touching.

The licensing coordinator observed nap cots and materials in the Toddler and Preschool rooms stored in such a manner that allowed the sleeping surfaces to touch. Corrective action: Stacked cots properly and placed extra blankets inside the cot so that no bedding was touching. We will make sure cots and blankets are put back correctly each time.

Programs shall maintain on file, available for review by the department, copies of current CPR and first aid certificates and licenses.

The licensing coordinator's review of three staff files revealed one staff member did not have a copy of their CPR and First Aid certification on file at the program. Corrective action: Place CPR certification in file on 4/30/24 was completely prior, just never placed in file folder. I will make sure to file paperwork right away.
