The licensing coordinator's review of five staff files revealed that two staff who are authorized to administer medication had not completed the training in medication safety and administration annually. Corrective action: All employees have now completed their medication administration training for the year 2026. Going forward, all staff will retake the training in April each year.

The licensing coordinator's review of the EOP and fire drill log revealed the program did not practice at least 2 different components of their EOP drills, other than evacuation drills, with all staff and children in 2025. Corrective action: We did not practice an EOP drill in 2025. We now have all EOP drills on our calendar to avoid this happening again and completed our first of the year EOP drill in January of 2026.

Child care staff shall ensure that potentially harmful items, including but not limited to matches, lighters, chemicals, materials labeled “harmful if swallowed,” flammable materials, sharp objects, and staffs’ personal belongings are locked or inaccessible to children.

The licensing coordinator observed soap labeled "keep out of reach of children except under adult supervision" on the sink, three cans of disinfectant labeled "flammable" on a shelf measuring 39 inches from the floor, and two boxes of Swiffer mopping pads labeled "keep out of reach of children" and containing cleaning chemicals on a shelf measuring 22 inches from the floor, in the bathroom of the PreK classroom. This bathroom is not located in the classroom and the staff confirmed that there are some children over the age of four that use this bathroom without direct adult supervision. The licensing coordinator moved these items out of reach of the children, since the class was outside at the time. Corrective action: All chemicals or items that can’t be in reach of children will be on the top shelf where the children can’t reach going forward. Our soap dispenser has soft soap which is safe for children but was in another type of soap dispenser. We have since bought new soft soap dispensers and got rid of any dispensers that are labeled keep out of reach of children.

Licensees shall have and implement written policies regarding: Children’s access to and use of television, video, and electronic devices.

The complainant's statement that the program allows children to use electronics during drop off times. Statements from the program's administration that they do not have a policy regarding the children's access to and use of television, video, and electronic devices. Corrective action: We have created an electronic / screen time policy and printed it out for all families and staff to review and sign. It will be added to our handbook when we do revisions this summer.

Applicants, licensees, and all child care staff shall keep confidential all records required by the department pertaining to the admission, progress, health, and discharge of children under their care and all information learned about children and their families.

Text messages provided to the department revealed that the program shared information about a child in the program with someone other than the child's parent or guardian. Corrective action: Staff were spoken to about always keeping information regarding children and families confidential. Our handbook was printed out for everyone to review our confidentiality policy. We also reviewed the Licensing Rule together. Going forward, no information will be shared with anyone other than the child’s legal parent / guardian.

Programs shall: Have a safe, functioning heating system.

The licensing coordinator observed space heaters in use in the program. The center director stated that they were having issues with their heating units so they obtained verbal approval from the fire inspector to use the use the space heaters until the heating units were repaired or replaced. Corrective action: I had verbal permission from both the fire department and the owner of the Mill to use the space heaters while the heat was being fixed on one half of our building. I received the written approval from the fire chief and sent it over to Licensing on February 5, 2026. Going forward, if I need to use space heaters again, I will get the written approval first.

In center-based programs, staff shall: Know the number and identity of children in their care.

The complainant reported that a two-year-old child was using the bathroom in the hallway without direct supervision from a classroom teacher. Staff D confirmed that a child younger than four years of age was allowed to leave the playroom to use the bathroom in the hallway without a staff member accompanying the child. Corrective action: Staff were spoken to about the licensing rule around leaving a child unattended in the bathroom. We spoke of how dangerous it is to allow a child under 4 to go to the bathroom alone. Going forward, no one under the age of 4 will be allowed to go to the bathroom unattended. Staff will call for coverage if alone with children.

Programs licensed to care for children younger than 3 years of age shall provide potty chairs or adult toilets with adapters.

The complainant reported that the program did not provide potty chairs or toilet seat adaptors for the adult toilets which made it difficult for toddlers who are toilet learning to use the toilet. Staff E confirmed that they did not have potty chairs or toilet seat adaptors for the adult sized toilets. Corrective action: We purchased a toddler potty seat option back in September of 2025 for easier use of the toilet for the younger children. Staff were spoken to about always having it available for a child if needed.

All center directors, agency administrators, site coordinators, or site directors, and all other child care staff who are responsible for the supervision of children, or who are necessary for the staff to child ratios, shall keep on file documentation of completion of a minimum of 6 hours of professional development, which shall be completed in accordance with the following: (1)  Within 90 days of the first date of employment; (2) Within 2 weeks for programs operating 3 months of the year or less; or (3) By providing documentation of previous completion.

The licensing coordinator's review of five staff files revealed that one staff did not complete the health and safety training regarding child development within 90 days of hire. Corrective action: Immediate action was taken. I spoke with the employee about finishing all trainings, and they completed the one she was missing on 8/18/25. I emailed my licensing coordinator a copy of the training that was completed. Going forward, I will look over the health and safety trainings in more detail to make sure they have all been completed.

The center director, site director, family child care provider, and all staff used to meet staff to child ratios shall: (1) Be certified in pediatric cardiopulmonary resuscitation (CPR) and first aid within 90 days of the first date of employment; (2) Obtain certification in (m)(1) above by the American Red Cross, American Heart Association, Emergency Care and Safety Institute, National Safety Council, or other nationally recognized organization; and (3) Maintain current certifications required in (m)(1) above.

The licensing coordinator's review of five staff files revealed that two staff did not maintain current certification required in pediatric CPR and First Aid. Corrective action: I have gone through each staff folder to see who is up to date on this. Whoever isn't was added to my list and I will be having someone come out to certify them. I have sent an email to the company that will come out to do the training. Once they get back to me, I will schedule that. I'm hoping to have this training scheduled as early as next week. Going forward, I will ensure they are up to date before hire and I will be offering CPR to all employees every 2 years.
