If any child has a serious injury while in the care of the program, consisting of fractures, dislocations, stitches, second or third degree burns, concussions, loss of consciousness, or requires emergency medical treatment or hospitalization, the family child care provider, center director, or site director shall (1) Immediately notify emergency personnel and the child’s parents; (2) Notify the department within 48 hours viaj e-mail, fax, or the NH Connections portal; and (3) Within 7 days, provide the department a written report via e-mail, fax, or the NH Connections portal, detailing the nature and circumstances of the serious injury.

The licensing coordinator’s review of injury reports revealed that on September 17, 2024, Child A sustained a serious injury while in care of the program. A review of department records revealed that the program did not notify the department of Child A’s serious injury within 48 hours of when they were notified that Child A received emergency medical treatment, nor did the program provide the department with a written report within 7 days of the serious injury, detailing the nature and circumstances of the serious injury. Corrective action: IN the future, the director will be sure to report any serious injury to CHild Care Licensing.

The licensing coordinator’s review of a sample of injury reports revealed that one report did not include the dated signature of the center director indicating that they reviewed the report. Corrective action: In the future, the director will be sure to sign all injury reports.

The program shall maintain daily child attendance records, in either paper or electronic format, including the child’s full name, date of birth, and their arrival and departure times in the program and each room or group throughout the day, as applicable, recorded in real time by staff or parents.

Child care staff and household members shall not use rough handling on children.

The program self-reported that on June 12, 2024, Parent A witnessed Staff A holding a child aggressively by their shirt, while yelling at the child, “We don’t put hands our hands on other people!”, then released the child, who fell backwards from a sitting position, hitting their head on the floor. Parent A’s statements concurred with the program’s self-report. Staff B stated that they took immediate corrective action upon learning of the incident. Corrective action: Staff A has been termintated effectively immediately. In the future, trainings and workshops will include topics such as "Handling Stressful Situations" and "Asking for Help" and "Positive Behavior Guidance". In the future, staff will be encouraged to ask other staff members for help when feeling frustrated or overwhelmed. Staff will be encouraged to take breaks as needed.

This Statement of Findings includes one repeat citation for He-C 4002.10(b) and one repeat citation for He-C 4002.21(u), which constitutes two repeat citations and failure to comply with one approved corrective action plan, as noted herein. The licensing coordinator's review of the daily child attendance records on the day of the visit revealed that one child’s full name and date of birth was not listed on the attendance, and in the Preschool classroom and seven children were signed in when eight children were present. The daily staff attendance records did not consistently include all staff’s full names, positions, or their scheduled work hours. Additionally, the licensing coordinator observed that the daily staff and daily child attendance records were not outside with the Infant and Toddler classroom and the Junior Pre-K classroom did not have their attendance in the classroom, when the record is required to be with each group throughout the day. Corrective action: New attendance sheets have been made for each classroom with full names and birthdates. In the future, each group will be sure to have their attendance sheets with them at all times, regardless of indoors or out. In addition, all staff members will be sure to sign in to the room that they are working in.
