For programs that administer medication: Programs shall not accept any prescription medications that do not include a prescription label or medication order from a licensed practitioner.

The licensing coordinator observed one EpiPen without a prescription label or medication order in the Caterpillar classroom. Corrective action: The original box with medication order had been placed in classroom backpack with Epi pen inside. In the future we will ask the parents/caregiver to provide the original box or the medication ( EpiPen) order form the Dr. on or before the first day of the Childs attendance.

The program shall obtain the following documents from the parents or guardians of each child with a food allergy or other allergy, which results in a serious reaction: A written care plan that includes instructions regarding food(s) or other allergens to which the child is allergic and steps for child care staff to take to avoid the allergens.

The licensing coordinator observed that there was no written care plan available for one child with a known allergy. Corrective action: A written care plan had been provided. The plan has been placed with the Child's Epi pen, in the class rooster board, in the child's file and in the confidential Allergy listing posted around the school (covered for confidentiality) In the future we will ask for the care plan in advance of child's first day of attendance. We will place the care plan with the epi pen and other posted areas. We will go over the care plan with the parent and staff that work with the child.
