The program shall obtain the following documents from the parents or guardians of each child with a food allergy or other allergy, which results in a serious reaction: A written care plan that includes instructions regarding food(s) or other allergens to which the child is allergic and steps for child care staff to take to avoid the allergens.
The licensing coordinator observed that there was no written care plan for one child with a known allergy. Corrective action: A FARE plan has been obtained for the child in question. In the future families will be required to provide a FARE plan upon registration for any child with an allergy.
