REPEAT CITATION PREVIOUSLY CITED ON JULY 11, 2023. The licensing coordinator observed two individuals in the program whose background record checks were expired. One individual worked 26 days and the other individual worked 20 days with an expired background record check. This constitutes the first repeat citation and failure to comply with one Directed Corrective Action Plan. Corrective action: Both individuals background checks are cleared and the program has conducted an immediate audit of all personnel files and implemented a formal compliance tracking system for all required background record checks. Moving forward, background check renewals will be initiated no less than 90 days prior to expiration, monitored monthly, and documented through an internal tracking process. A designated administrator will be responsible for oversight of licensing compliance, including monthly personnel file audits, renewal monitoring, and record maintenance. Administrative staff will receive retraining on NH Child Care Licensing requirements related to background record checks and staff file compliance. The program acknowledges the seriousness of this repeat violation and has strengthened internal controls to ensure no individual is permitted to work with an expired background record check, with all compliance documentation maintained and available for licensing review.
