The licensing coordinator observed two Aquaphor lotions, both expired 2/2025, in the bathroom between the Infant Two and the Toddlers classrooms, and a Desitin that expired 6/2023 in the Preschool classroom that were not disposed of properly by an authorized staff or returned to the child's parents or guardians. Corrective action: Medication boxes, areas of diaper creams, and other lotions are to contain the following spreadsheet. These spreadsheets are to be filled out upon the arrival of each new item. These areas are to be checked weekly for accuracy. These areas are also to be checked Monthly by a member of management as a form of a double check. This item has also been added to the weekly cleaning list of the classroom to be initaled by the teacher performing the check.

REPEAT CITATION PREVIOUSLY CITED ON FEBRUARY 18, 2025. The department received a complaint that on March 4, 2025, staff left a child outside on the playground for approximately 10 minutes. Staff A and Staff B confirmed that they inadvertently left a toddler outside when they went inside with the rest of the group. A review of video footage confirmed that the toddler was outside unsupervised for about 10 minutes before a program staff member found them and brought them inside. This constitutes one repeat citation and failure to comply with one approved corrective action plan regarding supervision of children. Corrective action: See attached document of CAP: Corrective Action Plan: Improving Teacher Accountability and Accuracy in Student Head Counts Issue Identified: Inconsistent and inaccurate student head count during transition from outside to inside, leading to potential safety risk and supervison gap. This will also be used in any transition from inside to outside, as well as any other movement from children from one space to another. Goal: Ensure all teachers consistently and accurately conduct student head counts, improving student safety and staff accountability. Teachers will now need to make sure to do a name to face count of each student moving from place to place. Teachers are to use following procedure: Pre-departure check of children matching face to name on attendance roster. They are then to do continuous counts during activity. Teachers are to perform a face to name check on return back to their class. Teachers are to make a verbal call to co-teachers to confirm number of children that left the area, as well as number that returned back. This will need to be double checked by co-teacher by a second walkie call to confirm number back to them to double check accuracy. This procedure will need to be supervised for the first two weeks of implementation to ensure accuracy and proper use of this format. The program self-reported that two children, both aged three years, were inadvertently left in the program's outdoor play area without supervision. A review of program documentation and interviews with program staff confirmed that the two children were unsupervised for 32 minutes before program staff noticed that the children were missing and located them in the outdoor play area. Program corrective action: Corrective Action Plan Investigating the February 10, 2025 Incident: Strengthening Safety Protocols On February 10, 2025, an incident occurred that highlighted a critical need in reviewing our safety procedures—ensuring that every child who leaves the playground re-enters the building. While the situation was addressed, it has underscored the need for a thorough review of our current protocols to prevent any future occurrences. Identifying the Issue During routine outdoor playtime, a discrepancy arose in the headcount when transitioning the children back inside. This event has prompted an internal review to refine our procedures and reinforce best practices. Strengthening Our Protocols To ensure that every child is accounted for during transitions, we are implementing the following proactive measures: Effective immediately: as of 2/10/2025. 1. Enhanced Check-In/Check-Out Procedures: Staff will use a two-step verification process when transitioning children between indoor and outdoor spaces. This will include a verbal roll call and a physical headcount at three points. Before we leave the class, at the door, and again at the gate. Not only counting, but also visual face checks. 2. Assigned Transition Roles: Specific staff members will be assigned the responsibility of overseeing playground transitions, ensuring that there is a clear chain of accountability. 3. Shadowing and Observation: We will conduct observational shadowing to assess the efficiency of our transitions, identify potential areas for improvement, and implement real-time solutions. 4. Clear Visual Markers: The use of designated entry and exit points, coupled with visual cues such as signs and markers, will help guide children and staff through safe transitions. Regular Training and Drills: Staff will undergo periodic training sessions to reinforce safety protocols and ensure consistency in practice. These will also occur sporadically for all classrooms throughout the day. Making sure that they can immediately respond with the number of children in their care. These immediate enhancements were put into effect, drafted up for all employees and shadowed and observed the following day to make sure that they were understood and adhered to.
