If an infant up to 12 months falls asleep outside of their crib or play pen, including entering the program asleep in a car safety seat, staff shall immediately move the infant and place them on their back in a crib or play pen.

During a conversation regarding infant nap locations, the program manager stated that infants may nap in the swing. Corrective action: If the infant falls asleep anywhere we will move them to a pack and play. We have 2 in the classroom and an extra one in the closest on top of the napping maps and can be taken out if all three infants are sleeping at the same time.

The program manager shall update the staff roster in the NHCIS portal within 5 business days when the individuals as described in (b) above are no longer working in the program or part of the household, with the date they left the program.

The licensing coordinator's review of the program's staff roster revealed that the program did not update the staff roster in the NHCIS portal within 5 business days of when two staff were no longer working in the program. The program manager confirmed that of the six staff on the roster one left the program in the Summer of 2024 and the other left in the Spring of 2025. Corrective action: I will make a note to exit out employees after they leave on the NHCIS web page. The two employees were exited out on the NHCIS page. So the list is current.

Child care staff shall ensure that for each child, upon the child’s first day in attendance in the program, there is a “Child Care Registration and Emergency Information” (April 2022) form or an equivalent form provided by the child care program completed and signed by the parent, on file, which contains: 1) Full legal name of the child; (2) Child’s date of birth; (3) Child’s physical address and mailing address; (4) Child’s home telephone number; (5) Date of enrollment in the program; (6) The name, physical address, and mailing address of the parent(s) responsible for the child, if different from the child’s address; (7) Telephone numbers for the child’s parents and instructions as to how the parents can be contacted during the hours that the child is at the program; (8) Email addresses for the parents, if available; (9) Names and telephone numbers of at least one person who will assume responsibility for the child, if for any reason, the parents cannot be reached immediately in an emergency; (10) Any chronic conditions, allergies, or medications in case of sudden illness or injury; (11) Written parental permission for first aid treatment; (12) Written parental permission for emergency medical transportation and treatment; (13) The name and telephone number of each child’s physician or health care provider; and (14) Names and telephone numbers of any person(s) other than parents who are authorized to remove the child from the program.

The licensing coordinator’s review of children’s records revealed the child care registration and emergency information form was not updated on an annual basis on two of the ten records reviewed. Corrective action: I had the 2 files updated the afternoon of the visit from child care licensing. In the future I will have all files updated the first week of January so that everything is current for that year.
