E. Bullock, owner/operator, was present and assisted with the visit. This family child care home currently operates with a Three-Star license issued on October 12, 2025, earning four (4) points in the Education Component, two (2) points in the Program Component and one (1) Quality point. Children were observed completing breakfast, engaging in circle time and activity center play. Observed interactions between the provider and children were nurturing in nature. The following violations of child care requirements were documented. Violation Number Comment Rule 908 Health questionnaire was not completed annually. Caregiver last health questionnaire was dated 1//1/2025. .1703(a)(1) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. There is no roster in the ABCMS provider portal for this program. G.S. 110-90.2 & .2703(r) 2015 Operator did not review the abusive head trauma and shaken baby syndrome policy with staff and/or the acknowledgement of the review did not contain required items or was not available for review. There was no signed acknowledgement with the required information for an enrolled child. .1726(d)(1-4) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by April 21, 2026. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter of compliance must detail how you plan to maintain compliance in the future in addition to how it was corrected. Please be aware that any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all submitted information is accurate and truthful. If it is determined that submitted information was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility’s license could be issued. COMPLIANCE HISTORY Prior to today’s visit your compliance history was 97% over an eighteen (18) month period and remains eligible for the child care license. OPERATOR RECORD I reviewed the operator’s file and observed one (1) violation. The last health questionnaire was completed 1/1/2025. This must be completed annually to ensure that the caregiver remains in good health and able to care for children. CHILDRENS RECORDS I reviewed children’s records and observed a violation. A child enrolled on 11/18/25 (E.H.) does not have an acknowledgement page signed and dated that they reviewed the SBS/AHT policy including the date of child’s enrollment. Rules require that signed acknowledgement, to include the child's name, date the child first attended the home, the date the policy was given/explained to the parent, parent's name and signature and date it was signed. QRIS RATED LICENSE TRANSITION You attended the QRIS: Pathway to the Stars meeting that was conducted by myself and another Consultant in Franklin County on 9/17/2025. Due to your facility having a license issued last year you will not be required to transition until the typical three (3) year period expires in 2028. TECHNICAL ASSISTANCE & REMINDERS Technical assistance given regarding ensuring that all plastic bags and diaper wipes are stored on the top shelf of the book case in the bathroom. This will ensure the items are five (5) feet above the floor and inaccessible to children. Continue to ensure that the outlets above the counter in the kitchen are covered when not being used. When parents do not note drop off or pick up times or signatures at the times they arrive then you must do so. Items discussed as technical assistance may warrant a violation if observed out of compliance. ABCMS PROVIDER PORTAL TRAINING I confirmed that you have completed the required ABCMS Provider Portal training by observing the training certificate in your file. Upon checking the ABCMS website there is no roster for your FCCH program on the site. While you are currently the only caregiver you must add yourself to the ABCMS roster for Lassiter’s Child Care in the ABCMS system. I emailed you guidance on how to do so. Please contact me if you have any issues and I can make the appropriate referrals. The Clean Water for Carolina Kids program has expanded to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. Currently operating child care facilities are required to test for lead in their paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. After checking the website, I noted that this program has completed the required lead water and lead paint screening/testing. However, the facility is listed as “Enrollment Started” for the asbestos testing. Please log into your account at the website (https://www.cleanwaterforuskids.org/en/carolina/) to check what steps need to be done to complete this sanitation requirement. The entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in one room or a section of the home, the license is issued for the entire location. Monitoring visits may include spaces other than those designated for care. A complete walkthrough will be complete at the annual compliance visit. Child care licensing requirements are established to ensure a safe and healthy child care environment. It is your responsibility to maintain compliance with all applicable laws and rules. The best way to do so is to periodically review the child care laws and rules. The current rule book dated July 1, 2025, is available at the DCDEE website https://ncchildcare.ncdhhs.gov/. There are also modules reviewing the new Child Care Rules available on Moodle, which can be found on the Division website. You, Earnestine Bullock, operator, were present and assisted me today. A temporary license was issued for this program on April 11, 2025, and is valid through October 11, 2025. The temporary license allows for first shift only and a capacity of 8 children, ages 0 to 12 years old. Restrictions on the license include daytime care only and a maximum of 5 preschool children at any time. The facility is owned by you as the sole proprietor. Contact me prior to making any ownership changes, such as developing a corporation. During this visit, we walked through both the indoor and outdoor environments and completed a full assessment of the licensed space. A variety of age-appropriate learning materials were observed in the child care area. You were caring for a total of five (5) children today. The children present ranged in age from infancy to seven (7) years of age. You and the children were in the room set up and designated for child care. The children were playing with toys and interacting with you. The children were served lunch before the conclusion of the visit today. I checked all rooms at the home today as well. Program records were also monitored today along with children’s records. Continue completing monthly fire drills and outdoor playground inspections. None of your information has changed since the Initial Assessment visit, except you completed the Recognizing and Responding to Suspicions of Child Maltreatment training. When asked, your email address is different than what was initially reflected in the Regulatory System. Your correct email address is: suggababieschildcare@yahoo.com. I corrected this in the Regulatory System during the visit today. Your phone number remains the same. The following violations were documented today: Violation Number Comment Rule 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. Documentation was not on file verifying the parents of six (6) children received a copy of the summary of NC Child Care Law. GS 110-102 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. Documentation was not on file verifying visual safe sleep checks were completed for two (2) infants for the following dates: June 23-27, 2025, June 30-July 3, 2025, and July 9-11, 2025. .1724(a)(8)&(f) The violations documented must be corrected immediately. Email or mail me verification the violations have been corrected on or before July 28, 2025. We reviewed what needed to be submitted for verification of correction. Your adult daughter arrived during the visit, and I also reviewed this information with her as you served lunch to the children. We were all present in the kitchen area while discussing this information. TECHNICAL ASSISTANCE & CONSULTATION: CHILDREN’S RECORDS: Children's records consist of various documentation such as a child's medical and immunization history, emergency medical care information, written plan of care, and parental acknowledgements. A health assessment/medical can be accepted if completed within the last 12 months prior to a child’s enrollment provided the information listed on DCDEE’s medical form is answered. We reviewed to maintain an acknowledgement statement on file verifying the parents/guardians of all children reviewed and received a copy of the summary of NC Child Care Law brochure. You made copies of the summary of NC Child Care law today for the parents to sign this afternoon when they pick up their children. VISUAL SAFE SLEEP CHECKS: Maintain at least the last 30 reporting days of visual safe sleep checks on file for review. There were some weeks that documentation was not on file for review verifying visual safe sleep checks were completed. We reviewed this and you stated you threw away some of the visual safe check documentation logs before realizing you were required to maintain the last 30 days on file for review. MONTHLY INSPECTIONS: Continue to complete the outdoor inspections must be completed and documented monthly on the Outdoor Playground Inspection Checklist for Family Child Care Homes form along with monthly fire drills. Completing a fire drill and monthly playground inspections on the same day is a great way to ensure compliance with this requirement. You can also set reminders with your phone or document when you plan to conduct a drill on a calendar. Ensure drills are conducted at different times each month. We discussed making sure both a fire drill and an outdoor playground inspection are completed this month and documented on the applicable forms. Use the smoke detector when conducting monthly fire drills instead of a whistle so children hear and get accustomed to the actual sound they will hear in the event of a fire. OUTDOOR PLAY AREA: You have completely fenced in the outdoor play area for the children with a vinyl coated chain link fence. We discussed using a plastic pool for the non-mobile children with putting a towel or blanket in it with some toys and materials for them to use when playing outdoors. Ensure you are positioned close to the younger aged children when outdoors. You have added a wooden play structure as well. Maintain the instructions from the manufacturer on file. A wading or swimming pool cannot be used for children in child care. You can use a sprinkler for water play. EMERGENCY PREPAREDNESS AND RESPONSE TRAINING: (EPR) Training: When asked, you have not yet completed the EPR training. This training is required to be completed within the first year of licensure. You are required to complete this training on or before April 11, 2026, based on when you were issued a temporary license. One the training is completed, your EPR plan must be completed via the Risk Management portal within four months after completion of the training. You will use your business NCID and password for the EPR plan. Once the EPR plan is completed, print the EPR plan and have it available for review. You will need to begin completing and documenting shelter in place or lockdown emergency drills every three months after completion of the EPR training. ON-GOING TRAINING: The ongoing training hours (in-service) will be monitored at your first annual compliance visit based on when you were initially licensed. No training hours were monitored today. The requirements are based on education and experience. You are required to complete ten (10) hours of training annually. You may count the health and safety trainings toward this requirement. Maintain copies of your training certificates on file for verification. You can also use the On-Going Training Record Log to document the trainings you complete and attach your certificates to the log. You are using the On-Going Training Record Log. You can attend any training offered by any Child Care Resource and Referral Agency in North Carolina. You can complete any training offered by the Southwestern Child Development Commission as well. RATED LICENSE: We discussed that you may voluntarily request to apply for a Two through Five Star Rated License at the end of the Temporary Time Period. You submitted a signed application on June 30, 2025, to be evaluated using the two component Rated License system at the end of the temporary time for the license. An accumulation of points will determine how many points will be earned for the Star Rated License. I will also bring you a copy of the Assessment for a Two Component Rated License. This form will be completed and signed during the next temporary time period visit. Contact me with any questions. EDUCATION INOFMRATION: You have created a WORKS account and per your education evaluation, you have a high school diploma, North Carolina Early Childhood Credential, and three (3) additional semester hours of ECE coursework. When asked, you are currently enrolled in an EDU course with Vance/Granville Community College and the course will be ending at the end of July. Once you have completed the course, request an electronic official transcript be submitted to the Workforce Education Unit at the Division of Child Development and Early Education. If the college does not participate with the National Clearing House, you will have to request an official transcript be mailed to you or you go by and purchase the official transcript from the college. Once received, you will have to mail the official transcript to the Workforce Education Unit at DCDEE for education evaluation. Education information must be completed and evaluated via the Workforce Education Unit for the coursework to be counted for the Rated License. PROGRAM STANDARDS: Once the facility is in compliance with applicable child care requirements, no additional action is needed to meet one point in this component. In order to earn two or more points, you will need to complete the 3-month long self-study and self-assessment using the FCCERS rating scale tool. In order to earn three or more points, you must have the FCCERS completed by the North Carolina Rated License Assessment Project and earn a specific score. I referred you to NCRLAP’s website, (www.ncrlap.org), for technical assistance and information regarding the 3-month self-study and self-assessment. You stated you have been working on a self-study, and you are maintaining documentation on file. Maintain documentation of completion of the three-month self-study on file for review and include dates when information is documented for my review during the final temporary time period visit. We discussed the FCCERS is voluntary and if not completed during your initial Rated License assessment you can request it later. As of February 1, 2025, the Family Child Care Environment Rating Scale was updated. The FCCERS has a third edition that is now being used as an assessment tool for programs that request the Scale as part of their Rated License assessment. QUALITY POINT: The various options for earning a quality point will be discussed during the next visit. COMPLIANCE HISTORY: It is very important that you maintain compliance with all applicable requirements at all times since you will be building a compliance history. In order to move to a star rated license at the end of the Temporary Time Period, you must have at least a 75% compliance history. Programs whose compliance history is below 75% could be issued an administrative action including denial of a license at the end of the Temporary License period. The violations documented today may affect your compliance history score. NC FAST: When asked, you have created an NC FAST account and are receiving subsidy for some of the children currently enrolled. CHILD CARE RULES: You can access the NC Child Care rules from the Division of Child Development and Early Education’s website: https://ncchildcare.ncdhhs.gov, click on the “provider” tab, then “child care rules” tab. Download a copy of the NC Child Care rules onto your computer so you have access to the rules and can reference them as needed. I forwarded you the most current Raise NC newsletter that was sent Friday, July 11, 2026. We briefly discussed information reviewed in the newsletter.
