A. Jefferson-Lewis, lead teacher, was present and assisted with the visit. Currently, this center operates with a Five-Star license issued on November 9, 2021, earning seven (7) points in the Education Component, seven (7) points in the Program Component and one (1) Quality point. The last annual compliance visit was conducted on January 17, 2025. The children in care were observed playing with age-appropriate toys and handwashing routines. Observed interactions between the teachers and children were nurturing in nature. The following violations of child care requirements were documented. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There was a wasp nest beneath the movable play equipment on the playground. Staff immediately called maintenance who came and sprayed the nest. 10A NCAC 09 .0601(a) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by October 7,2025. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter of compliance must detail how the violations were corrected. Please be aware that any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all submitted information is accurate and truthful. If it is determined that submitted information was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility’s license could be issued. OBSERVATIONS & STAFF/CHILD INTERACTIONS I observed a current activity plan and scheduled posted in the classroom. Upon my arrival the schedule was being followed as posted and the group was on the playground. There were a variety of toys to facilitate gross motor play such as tricycles, a sand box, space to run, play equipment, etc. There was a poster posted on the wall with each child’s name with circles where they were asked to identify their own names. The lesson for today included comparing the length and other attributes of pieces of ribbon. The children are still learning the schedule for the classroom, and the lesson plan notes review how to use items in the classroom environment. The children appear comfortable interacting with their teachers and classmates already. I observed a variety of age-appropriate materials to implement the lesson plan which includes activities for each of the developmental domains. There are two (2) children who have medication kept on site for chronic illness and all requirements were met. CLEAN CLASSROOMS FOR CAROLINA KIDS I reviewed the Clean Classrooms for Carolina Kids website regarding the required lead water, lead paint and asbestos testing. I observed that the program has completed the lead water testing as of June 19, 2025. They are listed as mitigation pending. The program has enrolled into the RTI lead paint and asbestos testing. The program is listed as “survey review by RTI.” This must be completed as soon as possible as the required enrollment date of May 31st has passed. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. ABCMS PROVIDER PORTAL I reviewed the ABCMS portal and confirmed that a staff roster is documented in the system for this program. This program has completed the required training and roster creation that satisfies the requirement related to notification of new staff hired within five (5) business days. TECHNICAL ASSISTANCE & REMINDERS Technical assistance discussed related to ensuring that staff/child ratios are maintained at all times. Please be reminded that the NC Pre-K staff/child ratios have changed and now staff/child ratios may be maintained of at most 1:10, during transitions or when items are needed from the an area outside the classroom or playground. Group size depends on the license and capacity of the space and must be maintained as stated on the license. A. Jefferson-Lewis, lead teacher, was present and assisted with the visit. Currently, this center operates with a Five-Star license issued on November 9, 2021, earning seven (7) points in the Education Component, seven (7) points in the Program Component and one (1) Quality point. The last annual compliance visit was conducted on February 1, 2024. The last sanitation inspection was completed May 3, 2024, earning a Superior rating. The last fire inspection was completed September 9, 2024 and is approved for daytime care. There is one NC Pre-K classroom operating at this facility and these requirements were monitored. I observed children participating in group time while the teacher read the book, “Pete the Cat and His Four Groovy Buttons,” which A. Jefferson-Lewis worked into their lesson topic of clothing. Interactions between teachers and children were observed to be comfortable and nurturing in nature. The following violations of child care requirements were documented. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). COMPLIANCE HISTORY Prior to today’s visit your compliance history was 96% over an eighteen (18) month period. FACILITY PROFILE This facility is owned by Franklin County Public Schools. I reviewed contact information for the owner and the facility with you and you verified the current information listed with DCDEE remains correct. STAFF RECORDS I reviewed new staff and ten (10) percent of current staff records, and two (2) violations were documented. The “Public School Off-Site Records Verification” form was completed for current NC Pre-K staff. The staff person, Nancy Alston (substitute) has an expired criminal background check expired 6/26/24 and there was no current qualification letter on file. Therefore, two (2) violations were documented. N. Alston has fifteen (15) days from today, which is February 1st, to complete the criminal background check and have a qualification letter on file. Until that is completed she may not work in the NC Pre-K classroom or be alone with children enrolled in the program including as a bus driver unless there is a bus monitor. CHILDREN’S RECORDS I monitored ten (10) percent or more of children’s records and they were found to be in compliance with Child Care Rules. NC PRE-K REQUIREMENTS The “Public School Off-Site Records Verification” form was completed for currently enrolled NC Pre-K children. This program operates from 7:45-2:45 daily and current staff are listed in the NC Pre-K plan and continue to meet requirements. This program uses Brigance as its developmental screening, and I observed a completed one for the record I monitored. Creative curriculum is an approved curriculum and in use at this program, the activity plan was posted and current. All applicable requirements were in compliance. TECHNICAL ASSISTANCE & REMINDERS Child care licensing requirements are established to ensure a safe and healthy child care environment. A. Lewis, lead teacher, was present and assisted in the completion of the visit. Currently, this center operates with a Five-Star license issued on November 9, 2021, earning seven (7) points in the Education Component, seven (7) points in the Program Component and one (1) Quality point. The last annual compliance visit was conducted on February 1, 2024. The last sanitation inspection was completed May 3, 2024, earning a Superior rating. The last fire inspection was conducted September 9, 2024, and the facility was approved for daytime care only. Children were observed engaging in activity center play, lunch routines including toileting and handwashing. Observed interactions with teachers and children were positive and nurturing in nature. This is an NC Pre-K classroom; however, NC Pre-K requirements will be monitored at the Annual Compliance visit. The playground will also be monitored at that visit due to active precipitation today. The following violation of child care requirements were documented. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. There was no authorization to administer medication form completed and on file for the two (2) children with medication on-site. 10A NCAC 09 .0803(4)(6-9) STAFF/CHILD INTERACTION OBSERVATIONS & TECHNICAL ASSISTANCE The school was on an early release schedule today, which means activities were completed at different times from the regular schedule. I did observe a current schedule and activity plan posted on the information board. The class was preparing for lunch when I arrived shortly after 10am. The children were dismissed around 11:20am. There were a variety of age-appropriate materials available for children to use and complete planned activities. The developmental domains, which help children to grow and learn in a healthy and age-appropriate manner, were easily identifiable on the activity plan. After lunch was completed, so were toileting and handwashing routines. The children then prepared their belongings to be dismissed. Children sat on the carpet and were given rhythm sticks and the teachers demonstrated how to use them while following the directions of three (3) songs played on a boombox. The children were able to tap the sticks together above them, in front of them and on the floor. The also followed song directions to march around the room. This was a great opportunity to work on some fine and large motor skills. They also practiced listening in order to follow directions. The teachers were both attentive and talked with the children frequently. MEDICATION There are two (2) children who have three (3) medications on-site, between them both, to treat chronic illnesses. While there are current medical action plans complete and available for both children there were no medication authorization forms completed and available to review. However, you were able to have both parents complete the forms shortly after dismissal and I observed them. Therefore, this was corrected during the visit. TECHNICAL ASSISTANCE & REMINDERS Continue to ensure that staff is documenting all arrival and departure times, with an adult signature or initials, if parents do not do so. Children’s arrival and departure most be noted as they occur and not later in the day, including those who arrive and depart via school bus. The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Go to https://www.cleanwaterforuskids.org/en/carolina/ for more information. STAFF/CHILD INTERACTION OBSERVATIONS & TECHNICAL ASSISTANCE A current schedule and activity plan was posted and observed being followed. There are a variety of age-appropriate materials available for children to use and complete planned activities. Activity centers were well supplied and being used by all children. Planned activities covered all the developmental domains which help children to grow and learn in a healthy and age-appropriate manner. There were various gross and small motor materials in the outdoor play area for children to use and create with such as a play house, tricycles, street signs, a mini trampoline, books and blocks. STAFF RECORDS There was a substitute working in the classroom as an Instructional Assistant (IA) during the visit. The substitute IA, L. Daniels, does not have a completed file on-site. However, the substitute IA did have a current criminal background check qualification letter on-site. We reviewed the substitute file checklist for centers which is found on the Division website noted below. I reminded you that any staff that works in the NC Pre-K classroom must have a substitute file on-site and available for review. You were informed of which documents must be completed and that substitute staff may be added to the Public-School Off-Site verification form regarding certain required documents. TECHNICAL ASSISTANCE & REMINDERS Technical assistance was discussed regarding documenting all arrival and departure times with an adult signature or initials. If parents forget to do so the teacher may and should documented children’s arrival and departure as they occur and not later in the day. Technical assistance also given regarding documenting incident reports on the incident log the same day, so staff does not forget to do so. Please remind other staff that cleaning products brought in by them must be kept in locked storage when not in use. The NC Commission for Public Health adopted and approved the amended rule regarding lead water testing effective October 1, 2019. The rule requires all licensed child care centers to test all drinking water faucets and food preparation sinks for lead contamination once every three years. If a child care center finds elevated concentrations of lead in the center’s water, they will be required to take immediate action. If your center was tested in 2020 you are due to re-test this year.
