This name of operation: monticello-brown summit elementary nc pre-k progra facility id: 41002132 consultant: wytina blackmon operation type: center case number: visit date: 4/2/2024 number present: 18 completed date: 4/2/2024 age: from 4 to 5 total minutes: 195 time in: 10:50 am time out: 02:05 pm time in: time out: list to use: center type of visit: annual comp full announced/unannounced: unannounced the purpose of today’s visit was to monitor for compliance during an annual compliance visit. a checklist was used to note the requirements i monitored today. this facility currently operates with a five star license issued on june 18,2018. upon my arrival to the facility, i was greeted by ms. s. williams, the lead teacher. i informed ms. williams the purpose of today’s visit. ms. williams gave me the necessary paperwork and assisted me during my visit today. all indoor and outdoor spaces used use by the children was observed today. all required items were posted. all medication and hazardous items were out of the children's reach. during today’s visit, i observed the children playing outside on the playground, washing their hands in preparation for the lunch, eating lunch, using the bathroom, napping, and departing. eighteen (18) children were present during today’s visit. a sanitation inspection was conducted on february 16,2024. this facility received a superior rating with four (4) demerits. staff and children files were monitored during today’s visit. the staff and training worksheets were updated during today’s visit. your signature on these forms serves as verification the information provided is accurate and complete. five (5) violations were cited during today’s visit. violation number comment rule 805 fire drills were not practiced monthly and/or the drill record was incomplete. there were no record of fire drills completed for the months may and june 2023. .0604(t); .0302(d)(5) 852 incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. the incident reports were not completed . the teacher stated she was not aware she need to complete the form if the children were not injured. .0802 (e) 853 incident logs were not completed and maintained as required. the teacher did not completed the incident form therefore couldn't add the incident to the log. .0802(g)(1-6) 859 monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. monthly playground inspections were not completed for the months april-june 2023. .0605(q) 1811 shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. the teacher did not practiced the required shelter-in-place or lockdown drills every three months. .0604(u);.0302(d)(8) the above violations must be corrected immediately, and a letter must be sent to me no later than april 16,2024 stating how each item was corrected. my email address is [email protected] if i do not receive your letter by april 16,2024 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. additional violations may be documented if the situation has not been corrected or addressed. consultation: today, i discussed with ms. williams the requirements for the incident form and log. you must complete the form with every incident. this will allow you to have documentation in case something serious happens form the incident. you will only need to send the form to your consultant if the child receives medical attention. you will have seven days to submit the form. rated license assessment: i also discussed with ms. williams the rated license assessment process will begin july 1,2024. i informed ms. williams this facility is in cohort 1. i requested this facility’s prep assessment on march 20,2024. ms. williams informed she has already been contacted by environmental rating scales scheduler and she will contact them to get her window. i informed ms. williams that we will wait for the facility’s score on the prep assessment before. your assessment year is july 1,2024 to june 30, 2025. education standards: today we discussed the education of the current staff. based on the information in our workforce database. the lead teacher meets seven points and the teacher meats three points in education. ms. b. clark the teacher stated that she had an associate degree and had just completed the cda certificate program. i informed ms. clark that based on her education in our workforce database system she only had a high school diploma. i assisted ms. clark with uploading her cda in the nc works system. thank you for your time. if the operator fails to correct any documented violations within the established time period, the division of child development and early education may deny, suspend, terminate, or revoke any permit to operate (10a ncac 09 .2000). all information in this report has been reviewed with me today.i understand that it is my responsibility to maintaincompliance with applicable nc child care requirements at all times inspection recorded no violations or advisories.

This name of operation: monticello-brown summit elementary nc pre-k progra facility id: 41002132 consultant: wytina blackmon operation type: center case number: visit date: 9/21/2023 number present: 18 completed date: 9/21/2023 age: from 4 to 4 total minutes: 150 time in: 10:45 am time out: 01:15 pm time in: time out: list to use: center type of visit: routine unannounced announced/unannounced: unannounced the purpose of today's visit has been to conduct a routine unannounced visit. upon my arrival, i was greeted by s. williams the lead teacher. i informed ms. williams the purpose of today’s visit. i observed and documented the attendance. eighteen children were present during today's visit. they were observed playing in centers, cleaning up, listening a story, washing their hands in preparation for lunch, eating lunch, using the bathroom and napping. limited licensing requirements were monitored. the followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; and permit restrictions. one (1) violation was cited during today’s visit. violation number comment rule 844 prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. one child's medication was not in the original labeled container. .0803(2)(a) the above violation must be corrected immediately, and a letter must be sent to me no later than october 5,2023 stating how the item was corrected. my mailing address is tina blackmon po box 1791 greensboro, nc 27402-1791 if i do not receive your letter by october 5,2023 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. additional violations may be documented if the situation has not been corrected or addressed. consultation: today there was a medication concern. we discussed the importance of receiving medication in the original box with the child’s name along with the doctor ‘s instructions. i told ms. williams that she could not receive any medication not in the original packaging because you will not know if it is the correct medication for the child. this will be a major liability for the school if this were administered not knowing if it was the correct medication for the child. ms. williams asked what the procedure for this type of situation. i informed ms. williams that she will need to inform the parents that she will need all required medication and documentation from the doctor prescribing the medication in order for the child to start. this will allow the parents to get the proper documents before the child starts. rated license: education: i discussed with ms. williams the process to get her education and ms. clarke’s education in the workforce education database. ms. williams told me that she had just completed the process of submitting ms. clarke's education information to workforce. program standards : i also discussed with ms. williams the rated license assessment process will begin july 1,2024 next year. there is an observation assessment that you can contact the rated assessment project. ms. williams stated that she would need the information for the observation assessment. i told ms. williams that i would send her the information when i return to the office. i informed ms. williams this facility is in cohort 1. you must let me know if you ready to start the process before june 30,2023. thank you for your time during today's visit. if the operator fails to correct any documented violations within the established time period, the division of child development and early education may deny, suspend, terminate, or revoke any permit to operate (10a ncac 09 .2000). all information in this report has been reviewed with me today.i understand that it is my responsibility to maintaincompliance with applicable nc child care requirements at all times inspection recorded no violations or advisories.
