A checklist was used to note the requirements I monitored today. This NCPK located in a public elementary school currently operates with a Five-Star License effective October 23, 2025. There is one classroom on site in use. The last annual compliance visit was conducted on 3/11/25. The facility has a 99% compliance history. Upon my arrival, I was greeted by the school office staff and signed in. S. Williams, lead teacher and Ms. B. Clarke, teacher greeted me upon arrival to the classroom. Fifteen (15) children were present during today’s visit. I observed the children playing outside, completing bathroom routines, eating lunch, and transitioning to rest time. All the teacher-student interactions I observed were positive and caring. A full assessment of childcare requirements occurred during today's visit. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. The NC Pre-K Program Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Staff and three children’s files were monitored. The staff and training worksheets were reviewed. Your signature on these forms serves as verification the information provided is accurate and complete. The sanitation inspection was completed 10/28/25 with a “Superior” classification. A satisfactory fire inspection was completed on 3/11/25. During today's visit, two items of non-compliance were cited. Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last documented playground inspection was completed on 10/17/25. .0605(q) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. Shelter-in-place or lockdown drills were not documented every three months. .0604(u);.0302(d)(8) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at [email protected] on or before March 5, 2026. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If I do not receive your letter by March 5, 2026, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the violations have not been corrected or addressed. Pathways to the Stars During today’s visit, information regarding the Pathway to Stars program and the updated rules in Section .3200 was provided. Instructions were shared for accessing resources on the DCDEE website. I informed you that the new rule rollout module is available on the DCDEE e-learning Moodle platform for review. The GCS Central Office Administrators have selected Program Assessment Pathway for this program. Your last ERS was conducted on 5/28/24. Your facility transition will begin in December 2026 and must be completed by May 2027.
