This NCPK located in a public elementary school, currently operates with a Five-Star License effective January 9, 2019. There are two classrooms on site. Upon my arrival, I was greeted by the school office staff and signed in. I introduced myself and stated the purpose of my visit. Ms. L. Jones and Ms. A. Porada are the lead teachers. Ms. Jones gave me the necessary paperwork I needed to complete today’s visit. Thirty-one (31) children were present during today’s visit. I observed the children participating in free choice and teachers directed activities, completing bathroom routines, eating lunch and transitioning to rest time. The classrooms are organized with many items for play and activities. A partial assessment of child care requirements occurred during today's visit. I monitored supervision, staff/child ratio, adequate/approved space, license posted and permit restrictions. Required documents were posted and /or accessible. Today five violations of child care requirements were cited. Violation Number Comment Rule 603 All floors and floor coverings were not constructed of nonabsorbent material and/or were not kept clean and in good repair. The area rug in Space 090 was observed to be significantly stained and may require cleaning or replacement. 15A NCAC 18A .2824(a)&(b) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A. Porada initiated the criminal background check on July 18, 2025, but the case was closed on September 15, 2025 without being completed. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. The qualification letter for A. Porada expired on August 18, 2025. G.S. 110-90.2(b) & (d) & .2703(e) 1864 Outdoor play equipment was not age and developmentally appropriate. There are 2 green teeter- tooters on the playground. Teeter-totters should not be used by preschool-age children unless they are equipped with a spring centering device and have an appropriate shock-absorbing material underneath the seats. .0605(a) 1865 The operator did not maintain the manufacturer's instructions on file electronically or in paper format for any outdoor play structures purchased or installed on or after September 1, 2017. The manufacturer's instructions for the two green teether totters were not on file electronically or in paper format. .0605(b) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at [email protected] on or before October 6, 2025. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. Date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If the letter of compliance is not received on October 6, 2025, a follow-up visit will be conducted. Today in Space #090: The qualification letter for A. Porada expired on August 18, 2025. According to the ABCMS system, A. Porada was previously qualified to work in child care on August 18, 2020. Qualification letters are valid for five years from the date of issuance. In ABCMS, it shows that the criminal background check process for A. Porada was initiated on July 18, 2025, but the case was closed on September 15, 2025, due to the Name Check Request not being submitted within the authorized timeframe. A. Porada will need to resubmit her information to restart the criminal background check process. Pathways to the Stars Transition Update The Pathways to the Stars transition will begin soon. A rule rollout module is now available on the DCDEE e-learning Moodle platform for your review. On September 8, 2025, the new rules outlined in Section .3200: Standards for Two through Five Star Rated Licenses were discussed with the school’s central office. The central office will determine which pathway your facility will follow. Today, we reviewed the three Pathways to the Stars, and I demonstrated how to access the relevant information on the DCDEE website. Thank you for your cooperation during today's visit. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
