R 400.8266(4) Incident, accident, injury, illness, death, fire reporting (4) A licensee, licensee designee, or program administrator shall report to the department, via phone, fax, or email, within 24 hours of notification by a parent that a child received medical treatment or was hospitalized for an injury, accident, or medical condition that occurred while the child was i n care.

The department did not receive any information regarding the incident with Child A that occurred on 12/09/2025 until 12/13/2025. An incident report was submitted in CCHIRP by the program administrator on 12/13/2025.
