Containers of poisonous or toxic materials were not kept out of reach of children. On 11/01/2021, 10 gallons of bleach, 4 large containers of liquid laundry soap, Goo-Gone Multi-purpose Cleaner Spray bottle cleaner, Clorox Fabric Disinfectants, sanitizing wipes, two bottles of floor cleaner, and other disinfectants were within reach of Child B.

The licensee, designee, or program director did not notify the department that a child was lost or left unsupervised. Ms. Chinavare and Ms. Gentry did not contact the department in writing within 72 hours of the incident. Ms. Chinavare and Ms. Gentry did not contact the department in writing regarding the incident that occurred on 11/01/2021.

On 11/16/2021 I received information that Child B walked out of a classroom and was missing for 5 to 10 minutes before being found. Child B was found hiding inside a laundry room, where several cleaning chemicals are stored. This incident occurred sometime between the end of October 2021 and early November 2021. I was unable to contact the complainant. No contact information was provided. The second incident occurred on 11/01/2021. Ms. Chinavare stated Ms. Senczyszyn came into her office around 11:45 am and asked her if she had seen Child B. Ms. Chinavare told Ms. Senczyszyn she had not seen Child B and immediately jumped up out of her chair and began checking classrooms, hallways, and areas outside of the building including the parking lot, playground, a neighboring business, and the general perimeter. However, they could not find Child B. Ms. Chinavare observed Ms. Senczyszyn try and open the laundry room door, but the door did not open, it was locked. Ms. Senczyszyn knocked on the laundry room door and called Child B but did not get a response. Ms. Chinavare stated that she and Ms. Senczyszyn were joined by other staff and the search ensued for around 5 minutes, to no avail. Just as she got ready to call 911 to report Child B missing, she turned around and observed Child B standing in the hallway next to the laundry room. Ms. Chinavare stated she ran down the hallway to get him. Child B told her he was in the laundry room because he wanted to dry his shirt. Child B was reminded that he is not allowed to leave 5 the classroom by himself for any reason and he cannot go into the laundry room. Ms. Chinavare said Child B did not sustain any physical injuries. Ms. Chinavare stated the laundry room door is always supposed to be locked and she was not sure why it wasn't. Ms. Chinavare stated that it is not uncommon for Child B to hide in the classroom. He is an active child and hides in a playful manner. Ms. Chinavare said staff placed bells on the door knob so staff could hear if Child B ran out of the room to hide. Child B's Mother was informed of the incident that same day that it occurred. I interviewed Ms. Senczyszyn. She stated the incident with Child B occurred on 11/01/2021 during morning worktime around 11:45 am. She said Child B painted at the easel then started painting the drying rack and floor. Ms. Senczyszyn told Child B to paint on paper, not on the drying rack and floor. Child B helped her clean up. In the process of cleaning, Child B's shirt got wet. Ms. Senczyszyn told him to get a clean shirt from his cubby in the hallway. Child B walked out of the room. Ms. Senczyszyn went to the sink and washed and dried her hands. She observed that Child B had not returned to the classroom, so she walked out of the room to check the cubby area. She did not see him. Ms. Senczyszyn yelled for Child B and asked Ms. Nahas if she saw him anywhere. She had not. Ms. Senczyszyn began checking other classrooms and told Ms. Chinavare that she could not find Child B. Ms. Senczyszyn said that she, Ms. Chinavare, and other staff looked inside and outside of the center. She recalled knocking on the laundry room door and calling out to Child B but received no response. A short time later, Ms. Chinavare found Child B outside the laundry room and returned him to class. Ms. Senczyszyn stated that Child B is a "runner," and that it was common for him to hide in other rooms around the center. Ms. Senczyszyn stated that since the incident she moved the cubbies inside the classroom and has put up a gate to prevent Child B from escaping the room. Ms. Senczyszyn stated she told Child B's Mother about the incident the date it occurred. On 11/16/2021 I observed the laundry room at the center. The laundry room is a hallway adjacent to the preschool room 6 hallway. The laundry room hallway and preschool room 6 hallway meet about 10 feet from the preschool room 6 classroom. The laundry room is about 20 feet away from the location where the two hallways me
