On 12/09/2020 in the early preschool classroom, Marissa Isaac picked up Child A by the arm, carried him by the arm across the classroom, and placed him in a chair.

On September 30, 2019 and October 1, 2019 caregiver Tasneem Hussein provided inappropriate care and supervision to Child A (3- year-old male), Child B (3-year-old female) and Child C (3-year-old female) when she pulled them by their arms to direct them. On October 1, 2019 caregiver Tasneem Hussein grabbed Child A by the arm and pulled him backwards, resulting in him hitting the back of his head on the floor. The Montessori Room is out of ratio.

In the School-Age room, one child’s levocetirizine dihydrochloride and one child’s epi-pen was not in the original container.
Two of the nine staff did not complete administration of medication training within 90 days after being present at the 3 LANDMARK BUILDING • 105 W. ALLEGAN STREET • LANSING, MICHIGAN 48933 Michigan.gov/MiLEAP center and before caring for children unsupervised. One staff was hired on 07/29/2024 and one staff was hired on 01/27/2025.
The hand washing sink in the Preschool room’s bathroom was 121 degrees Fahrenheit. The other hand washing sink in the Preschool room was 123 degrees Fahrenheit.
When I arrived at the child care center for the interim inspection at 10:30 a.m., there were 30 children in the Pre- Kindergarten room. The room capacity is approved for 20 children.
Based on interviews with child care staff members, classroom observations, and review of incident reports, child care staff members provide appropriate care and supervision of children in the Two's classroom.
Based on the interviews that were conducted, there is evidence to support that CCSM 1 and CCSM 2 did not provide appropriate care and supervision when Child A was left unsupervised in the classroom from 9:41 a.m. to 9:45 a.m. CCSM 1 and CCSM 2 did not follow the child care center’s face to name policy.

Two of the three children who used the Kindervan buggy on 05118/2021 were under the manufacturers recommended age of . Child A and Child B were age and respectively.
Consistent with R 400.8158(3). Ms. Black notified me of the accident on 5119/2021 and provided an incident report. While a staff member was 0puushing Child A back into the classroom in a buggy on 5/1812021, the child stuck out hand and it was pushed back slightly. The parents were contacted because was not using arm. arents took to the where Chi A was diagnosed with a fracture. _ and I interviewed Nya Flannigan-Tucker, infant lead caregiver, at the center on 05/25/2021. She said she was headed outside with three children in the buggy. She forgot Child A's sunscreen and returned to the classroom. She said it is a tight fit through the doorway. When she noticed Child A stick out arm and saw it bend, she immediately pulled the buggy backward to release it. A d A was crying. She called administrative assistant Kathleen Sarna to report the accident. Ms. Black was also present at the center. Ms. Sarna said she gent) felt Child A's arm. It seemed tender. Child A cried harder. She immediately called parents. She believes she first reached father who asked them to monitor i and call if there were any changes. Ms. F anni an-Tucker attempted to feed Child A a bottle with little success. Ms. Sarna called parents again and both arrived about 30 minutes later. and I interviewed Kaitlyn Cringer who has worked with Ms. Flannigan- Tucker. She described her as being very kind and loving with the infants and knowledgeable of their individual needs. Ms. Cringer said she has no concerns about Ms. Flannigan-Tucker's interactions with children. Ms. Flannigan-Tucker said she was the sole child care staff member with a total of three children who were determined to be Child A, Child B, and Child C who were age , nd , respectively. on 05/18/2021. She said the chit ren werea s"rappUin. Pace forward, when the incident occurred. and I looked at the four-seated buggy equipped with adjustable 5-point safety harnesses during our visit. The buggy appeared to be in good condition. Per Ms. Black, the safety plan to help prevent the accident from recurring includes having staff push children through doorways with their backs facing the direction of movement when entering and exiting the classroom. watched a video of the incident recorded by the center on 05/2812021. - Neither of us found any evidence that the injury was anything other than an accident.
On 12/10/2020, I was forwarded an incident report from Ms. Black. The incident report stated that the center received a telephone call on 12/09/2020 at around 2:45pm. The call was from a parent who witnessed an incident in the early preschool classroom while using the WatchMeGrow application. This application allows parents to view cameras in the center's classrooms. The parent had observed an inappropriate interaction between Ms. Issac and Child A. The parent saw Ms. Issac pick up Child A by the arm and place Child A in a chair. On 12/15/2020, I spoke with Child B's mother via telephone. Child B's mother stated that on 12/09/2020, sometime after 2:00pm, she logged onto the WatchMeGrow application. Child B's mother indicated that she often does this throughout the day to check in on Child B. Child B's mother stated that she observed the children getting up from nap and putting away their nap supplies. Child B's mother saw two caregivers in the classroom. One caregiver appeared to be cleaning up the cots and the second caregiver was interacting with the children. Child B's mother stated that the second caregiver was talking aggressively to a group of children. There is no sound on the application, but the caregivers body language made it appear that she was upset and frustrated. The caregiver then walked over to some of the children and grabbed Child A by the arm forcefully. The caregiver picked Child A up by his arm so that he was completely off the floor. The caregiver dragged Child A over to a chair and forcefully used her body to push him down into the chair. Child A appeared very upset and was crying. Child B's mother was so disturbed by what she saw that she immediately contacted the center to inform them of what she witnessed. Child B's mother requested that a staff member be sent to the classroom to talk with the caregiver. Child B's mother stated that she has never had past concerns regarding staff members interacting in an inappropriate manner towards the children. Child B's mother stated that she was surprised and taken off guard when she saw the caregiver acting this way towards Child A. Child B's mother stated that they are not allowed to go into the center due to the corona virus pandemic. Child B's mother stated that she does not know the caregivers as much as she did pre-pandemic since she does not interact with them daily. Child A is no longer enrolled in the center. On 12/16/2020, I conducted an announced on-site inspection. I meet with Tanya Zoro (district manager) and viewed the video recording of the incident. The video recording does not have audio. The video recording showed the early preschool classroom on 12/09/2020 at approximately 2:35pm. Ms. Isaac can be seen talking with a group of children. Ms. Issac is pointing towards the desk and appears to be 3 verbally scolding the children. Ms. Issac walks forward and leaves the view of the camera for a few seconds. When Ms. Issac reappears, she is carrying Child A by the arm. Ms. Issac is using her right arm and is holding Child A by his left arm. Ms. Issac is carrying a pile of blankets under her left arm. Ms. Issac is carrying Child A in front of her body and he is slightly angled since she is using her right arm to carry him by his left arm. While Ms. Issac walks across the classroom, Child A is completely off the floor and is being carried solely by his left arm. Ms. Issac sets Child A on the floor by a table, and then picks him back up and physically places him in a chair. Child A is visibly upset and can be seen crying. Ms. Issac then leaves Child A in the chair and walks away. While on-site I observed the early preschool classroom and saw the area in the classroom where the incident occurred. I interviewed Kimberly Black (program director) and Gail Zelyez (lead caregiver-early preschool). Ms. Black stated that on 12/09/2020, she returned a call from Child B's mother around 2:40pm. Child B's mother reported that she witnessed a caregiver lifting Child
I received a telephone call and written incident report from district manager, Lisa Potvin. Ms. Potvin stated that on October 1, 2019 at approximately 5:18 PM, Child A's Mother called to report concerns regarding the interaction between Child A and caregiver Tasneem Hussein on September 30, 2019 and October 1, 2019. On Monday, September 30, 2019, Child A's Mother was watching the center's live feed video and observed caregiver Tasneem Hussein roughly grab Child A who was sitting down and crying. Ms. Hussein came over to him, started pointing at him and made him go clean up his bedding. Child A's Mother said she was very upset by what she saw because it looked like he was punished for crying and was made to go clean. Child A's Mother called the office and asked to speak with the director. She spoke with someone (name unknown) who stated the director was not present at the time. She did not leave a message. On Tuesday, October 1, 2019 Child A's Mother watched the live feed video and became concerned when she observed Ms. Hussein grab the hand of Child B and push her down to sit. After this, she saw Ms. Hussein roughly stop Child A, drag him over to a spot on the floor and very roughly "tossed" him down on the floor. It looked like he hit his head on the floor as a result of Ms. Hussein's actions. Throughout the day, she observed a few more incidents that were very aggressive tactics of dealing with the children that involved grabbing and pulling of the children's hands in order to deal with them and get them to listen. Child A's Mother called the center's corporate office after not being able to reach anyone in management at the center's location to report it. The corporate office followed up with her later that night and the next day saying they will put Ms. Hussein on administrative leave and will review the footage. Child A is no longer enrolled at the center. Child A's Mother made police report #19- 17728; was assigned to this case. Ms. Hussein was placed on administrative leave the day after Child A's Mother expressed her concerns. During the onsite inspection, Wendy Donovan, Lisa Potvin, Tiffany Raby, Kim Black, and myself watched the videos of the incident. i In the video on September 30, 2019, I observed Tasneem Hussein with 14 three- year old and four-year-old children in the Montessori Room. Per the face to name sheet, 11 children were signed in with 1 caregiver between the hours of 2:40 pm and 3:47 pm. This room was out of ratio and needed one additional caregiver. In the video dated September 30, 2019 I saw caregiver Tasneem Hussein point at Child A, who was sitting calmly on the center's carpet near his mat and blanket after naptime. Ms. Hussein walked over to him and grabbed him by the upper arm. She pulled him into a standing position by the upper arm. She pointed to Child A's mat and blanket; Child A walked over to the mat and started cleaning. I did not see him crying at any point in the video. I reviewed the video dated October 1, 2019; there was no time stamp on the video. I observed Child A seated in the upper right corner of the carpet. Ms. Hussein reached over to Child A by the arm from the corner of the mat to the center of the mat. She continued to stand over Child A, but I could not see the rest as it happened off camera. In the next segment, I observed Child A seated on the upper right side of the carpet. Ms. Hussein reached over to Child A by the wrist and pulled him up from a seated position to a standing position. She pulled him over to the left side of the carpet and 4 gave his arm a push, causing him to sit down hard on the carpet. Child A was not crying during this incident. In a separate incident, Child B (3-year-old female) was standing in the back of the room near the activity center holding her blanket. Ms. Hussein grabbed Child B's wrist and pulled her behind the activity center and grabbed the blanket out of her hands. Child B did not appear upset and was not crying during this incident. The
The lock on the cabinet under the sink in the Preschool room was broken. There was a bottle of bleach solution, sanitizer, and toilet bowl cleaner that was accessible to children.
When I arrived at the child care center for the interim inspection at 10:30 a.m., there were two staff supervising thirty children in the Pre-Kindergarten room. The age range for the Pre-Kindergarten room is 3 years of age until 4 years of age with a staff-to-child ratio of 1 to 10.
In the School-Age room, there was no written permission form for one child’s epi-pen.
In the Toddler 1 room, there was a bottle of Ripple Kids Plant Based Milk and a carton of Soy Milk that did not have the children’s first name, last name, and date that it was opened.
The licensee failed to maintain ratio in the Montessori Room: • On September 30, 2019 caregiver Tasneem Hussein cared for 14 three-year old and four-year-old children in the Montessori Room by herself. • On October 3, 2019, the Montessori Room was out of ratio from 12:37 pm to 3:00 pm with one caregiver present with 13 preschoolers between the ages of S- ears-old and 4-years-old.
