I received a telephone call and written incident report from district manager, Lisa Potvin. Ms. Potvin stated that on October 1, 2019 at approximately 5:18 PM, Child A's Mother called to report concerns regarding the interaction between Child A and caregiver Tasneem Hussein on September 30, 2019 and October 1, 2019. On Monday, September 30, 2019, Child A's Mother was watching the center's live feed video and observed caregiver Tasneem Hussein roughly grab Child A who was sitting down and crying. Ms. Hussein came over to him, started pointing at him and made him go clean up his bedding. Child A's Mother said she was very upset by what she saw because it looked like he was punished for crying and was made to go clean. Child A's Mother called the office and asked to speak with the director. She spoke with someone (name unknown) who stated the director was not present at the time. She did not leave a message. On Tuesday, October 1, 2019 Child A's Mother watched the live feed video and became concerned when she observed Ms. Hussein grab the hand of Child B and push her down to sit. After this, she saw Ms. Hussein roughly stop Child A, drag him over to a spot on the floor and very roughly "tossed" him down on the floor. It looked like he hit his head on the floor as a result of Ms. Hussein's actions. Throughout the day, she observed a few more incidents that were very aggressive tactics of dealing with the children that involved grabbing and pulling of the children's hands in order to deal with them and get them to listen. Child A's Mother called the center's corporate office after not being able to reach anyone in management at the center's location to report it. The corporate office followed up with her later that night and the next day saying they will put Ms. Hussein on administrative leave and will review the footage. Child A is no longer enrolled at the center. Child A's Mother made police report #19- 17728; was assigned to this case. Ms. Hussein was placed on administrative leave the day after Child A's Mother expressed her concerns. During the onsite inspection, Wendy Donovan, Lisa Potvin, Tiffany Raby, Kim Black, and myself watched the videos of the incident. i In the video on September 30, 2019, I observed Tasneem Hussein with 14 three- year old and four-year-old children in the Montessori Room. Per the face to name sheet, 11 children were signed in with 1 caregiver between the hours of 2:40 pm and 3:47 pm. This room was out of ratio and needed one additional caregiver. In the video dated September 30, 2019 I saw caregiver Tasneem Hussein point at Child A, who was sitting calmly on the center's carpet near his mat and blanket after naptime. Ms. Hussein walked over to him and grabbed him by the upper arm. She pulled him into a standing position by the upper arm. She pointed to Child A's mat and blanket; Child A walked over to the mat and started cleaning. I did not see him crying at any point in the video. I reviewed the video dated October 1, 2019; there was no time stamp on the video. I observed Child A seated in the upper right corner of the carpet. Ms. Hussein reached over to Child A by the arm from the corner of the mat to the center of the mat. She continued to stand over Child A, but I could not see the rest as it happened off camera. In the next segment, I observed Child A seated on the upper right side of the carpet. Ms. Hussein reached over to Child A by the wrist and pulled him up from a seated position to a standing position. She pulled him over to the left side of the carpet and 4 gave his arm a push, causing him to sit down hard on the carpet. Child A was not crying during this incident. In a separate incident, Child B (3-year-old female) was standing in the back of the room near the activity center holding her blanket. Ms. Hussein grabbed Child B's wrist and pulled her behind the activity center and grabbed the blanket out of her hands. Child B did not appear upset and was not crying during this incident. The
