There were several chipped off paint areas in all the classrooms. The exit door in the school age classroom has chipped off paint on the door.
The outdoor play area does not have sufficient ground covering under the play structures. Many of the push toys in the outdoor play area are broken. The bridge in the climber is broken.

On 6/7/22, I interviewed the complainant who stated that some of the push toys used by the children in the outdoor play area were broken. On 6/9/22, I conducted an onsite inspection at the center. I observed the outdoor play area. There were several push toys and none of them were broken or needed repair. On 6/9/22, the lead caregiver, Ms. Julian, who was in charge while the program director was out stated that the toys that were broken or need repair are usually thrown out or not used by the children.
On 6/7/22, 1 interviewed the complainant who stated that this summer the ground covering under the play structures is not sufficient. The complainant said that the children play a lot on it and the wood chips were depleted. On 6/9/22, I conducted an onsite inspection at the center. I observed the outdoor play area and saw that there was sufficient ground covering under the climbing play structures used by toddlers and preschool children. The licensee designee, Ms. Breakie-Graybill stated that more woodchips would be delivered on 6/15/22 and it will be spread on the playground on 6/16/22. 2
The center has a plan for how and when a parent is notified when personnel observe an experience an accident, injury, or incident. On 08/24/2021, Staff did not observe the child experience an accident, injury, or incident occur at the center; therefore, the parent was notified.
The program director was present less than 6 continuous hours at the center. Ms. Breakie-Graybill informed me that she was not meeting this rule requirement. The observed schedule indicated that she worked less than 6 hours a day (on some days) and less than 30 hours a week.
On 08/26/2021 I interviewed Child A's Mother. She said on 08/24/2021 she picked up Child A from the Breakie Bunch. When she picked up Child A, she observed her face and body were covered in chalk and dirt. Child A's Mother said she asked child care staff member Deborah Wilson about Child A's day, but Ms. Wilson did not mention anything about an injury to Child A. She simply stated that Child A had a good day. Child A's Mother stated once she got home and bathed Child A, she observed a 1/2 inch to inch gash near her left eyelid and a bruise above her left eyebrow. Child 3/4 A's Mother asked Child A what happened. Child A said that she hit her head on the shelf when she bent down to pick up a toy triangle. Child A told her mother that she told child care staff member Cassie Williamston that she hurt herself. Child A's mother was very concerned about the injury to Child A's eye area and stated she is typically notified of an injury on the center's parent communication app, or she is given a hard copy of an accident/injury report when she arrives at the center to pick her child up. However, neither of these things happened. Child A's Mother stated this is not the first time this happened at the facility either. She has tried to make contact with the Program Director, Laura Breakie-Graybill, but she is never working at the center. Child A's mother is concerned with the lack of supervision and the training that staff received when they begin working. She 3 indicated that child care staff are not being trained properly on center polices and protocols. On 08/26/2021 I received three photographs of Child A's left eye from Child A's Mother. There is a inch to inch gash and a large purple bruise above and below 1/2 3/4 her eye. Each photo displayed was similar. One was a close-up and the other two were taken at different angles. On 08/26/2021 I conducted an announced onsite inspection at the Breakie Bunch. I interviewed Ms. Breakie-Graybill. Ms. Breakie-Graybill stated that Child A's Mother asked her about Child A's injury to her left-eye area. Ms. Breakie-Graybill stated that Child A never told anyone she was hurt, and nothing was noticed by staff. She said it is the center's policy to notify parents through the center's communication app, a phone call, or paper copy of the accident/injury report when an accident or injury is observed. She said Child A's Mother told her that she was upset about the injuries occurring at the center and that staff are not aware of what happened. She felt that staff may not be paying close enough attention to children. She told Child A's Mother she would follow up. The next day, Ms. Breakie-Graybill asked Ms. Wilson to complete an incident report for Child A. Ms. Wilson wrote the report and gave it Child A's Mother. However, Child A's Mother refused to sign the incident report, stating Ms. Wilson is not the responsible party. Ms. Breakie-Graybill stated she could not recall any staff telling her that they observed Child A being injured. It is not clear if the injury occurred at the center. She stated the classroom was in ratio. There were 8 children with 1 child care staff member. I received a copy of a rollcall sheet for the preschool classroom. There were 8 children signed into the preschool classroom on 08/24/2021. All children were between 3 and 4 years old. Ms. Breakie-Graybill indicated that sadly there has been a lot of staff turn over in the past few months, but she has been working hard to maintain consistency for staff. She stated she is using a substitute staffing program. This program provides qualified staff to work in child care. The program is called Sub Hub. She is using subs to cover more than one vacant position. She stated that training new staff (not substitutes) has been difficult, especially since the center is short staffed. Typically, new staff will receive a team training manual and will undergo 5 days of comprehensive training. Each training day provides specific center policies a
On 6/7/22, I interviewed the complainant who stated that there was a short climber in the outdoor play area which had a broken bridge on it. It was not safe for the children. On 6/9/22, I conducted an onsite visit at the center. I observed the climber that had a broken bridge. I pointed it out to Ms. Julian and asked her to make sure that the children do not use the climber until this is repaired. I did not see any children playing on it during my inspection. Ms. Julian agreed to not allow children to play on the climber. On 6/15/22, licensee designee, Ms. Breakie-Graybill called to inform me that the structure was disassembled and thrown out on 6/10/22. In the meantime, children did not play on it. I provided technical assistance to Ms. Breakie-Graybill to make sure that the staff members check the equipment hazards on a regular basis before letting the children play on it.
The center did not provide an orientation about the center's policies and practices and administrative rules before unsupervised contact with children. Ms. Breakie-Graybill and Ms. Wilson stated that staff did not have a complete orientation before unsupervised contact with children.
