One of the three child care staff members (CCSM) did not complete child development training within 90 days of being hired at a child care home. The CCSM was hired on 11/05/2025 and she has not completed the training.
Licensee did not accurately and maintain the connection status of one current CCSM and the disconnection status of two former CCSMs and one unsupervised volunteer in the child care background check (CCBC) system.

There was no comprehensive background check completed for child care staff members prior to contact with children. Staff are not required to provide proof to indicate that required training was completed prior to contact with children. I reviewed all staff training records that the licensing rules require before a staff member can have contact with children. The health and safety courses taken by Ms. Miller and Ms. Williams satisfied the training needs as detailed below, but only Ms. Miller completed the training prior to hire in 2018. Ms. Williams was hired in January 2020 and completed the training in August 2020. Ms. Glass only had verification of completing Health and Safety Course 1 provided by MiRegistry and a refresher course. She took separate trainings prior to hire in safe sleep and infectious disease as detailed below. Regarding CPR and first aid training I reviewed staffs' documentation on site. Ms. Glass, Ms. Williams, and Ms. Miller all had current certification in infant, child and adult CPR as well as first aid. All were dated prior to hire. 3 Regarding the prevention of infectious disease, Ms. Miller satisfied this need prior to contact with children when she completed Health and Safety Course 1 on 08/21/2018. Ms. Williams completed this training on 11/12/19 prior to contact with children. Ms. Glass satisfied this requirement when she completed MiRegistry Health and Safety Course I on 10/24/2020, prior to contact with children. Regarding shaken baby syndrome and safe sleep, Health and Safety Course 2 taken by Ms. Miller on 08/21/2018 before contact with children included shaken baby syndrome training but she did not have verification of training in safe sleep on file. Ms. Williams did not have verification of shaken baby syndrome training prior to contact with children but did have documentation of safe sleep training on 04/01/2019 which was taken before she had contact with children. Ms. Glass did not have evidence of training in shaken baby syndrome or safe sleep before coming into contact with children. Regarding reporting child abuse and neglect, Ms. Miller had this training when she completed Health and Safety Course 2 prior to hire. The licensee was specifically cited when Ms. Williams did not have evidence of completing child abuse and neglect training prior to hire in a Licensing Study Report dated 07/21/2020. She has since completed this training (08/04/2020). Ms. Glass did not have verification of abuse and neglect training prior to hire. training. (2) Before caring for children at a child care home, an individual shall provide the licensee with all of the following: (a) A valid certification in infant, child, and adult CPR. ANALYSIS: All staff had current certification in infant, child, and adult CPR prior to caring for children. .................... (2) Before caring for children at a child care home, an individual shall provide the licensee with all of the following: (c) Proof of training in the prevention of infectious disease, including immunizations. Hours of training in the prevention of infectious disease from MiRegistry will be allowed to count for training hours to meet this requirement. ANALYSIS: All three staff members had training in infectious disease prior to hire. CONCLUSION: VIOLATION NOT ESTABLISHED APPLICABLE RULE R 400.1905 Training. (3) A licensee shall ensure that child care staff members and child care assistants, prior to caring for children, have training that includes information on all of the following: (a) Safe sleep practices to prev Screening for communicable tuberculosis (TB) is not completed before staff have contact with children. The cabinet doors in the kitchen were broken and falling off with the screws sticking out.

This interim inspection recorded no violations or advisories.
On the inspection date, I looked at the cabinets on the lower level of the home which is approved for children's use. The cabinets were secure and in good condition with no protruding screws.
The swings have a play surface that reaches above 30 inches and were mounted above a wooden deck. ............

I called and reached the complainant on 08/25/2021. She said she was unable to speak to me at the time. I asked for but did not receive a call back. I made an unannounced on-site visit to the home and interviewed licensee Jayeeta Champati. She identified Tina Glass, hired three days ago, and Kayla Williams, hired 01/02/2020, as her only staff members as of the inspection date. She said the other recent hire was Tori Miller on 06/18/2021. She is no longer employed. I briefly interviewed Ms. Williams and Ms. Glass before Ms. Champati returned to the child care home. The approximate dates of hire were consistent with what the licensee reported. They believed they had completed all required pre-employment needs, except that Ms. Glass said she had not yet obtained a TB test. Afterward, I 2 reviewed their employee files and LARA's database. Tina Glass was hired on 8/22/21. The Child Care Background (CCBC) database Indicates that a comprehensive background check was completed and that she was determined eligible to work with children on 08/18/2021, prior to coming into contact with children. Ms. Williams was hired 01/02/2020. The CCBC database indicates that she was determined eligible to work with children on 12/28/2019, prior to working with children. Ms. Miller's date of hire was 6/18/2021. The CCBC database indicates Ms. Miller was found eligible to work with children on 06/16/2021, prior to contact with children. Only Ms. Williams had been employed for more than 90 days.
The licensee did not have documentation to verify that Ms. Williams had training in the prevention of shaken baby syndrome, abusive head trauma, and child maltreatment prior to caring for children. I saw documentation of the TB tests. Ms. Miller obtained it on 8/15/17, prior to hire. Ms. Williams obtained it on 11/08/2019, prior to hire. Ms. Glass said she had not yet obtained a TB test.

The licensee did not have documentation to verify that Ms. Glass was trained in child abuse and neglect prior to hire.
The kitchen cabinets are in good repair and maintained in a I clean, safe condition.
The swings and both climbers were not placed at least 6 feet from the perimeter of other alav structures or obstacles.
