I Interviewed Lori Todd by telephone on 11/04/21. On 11/03/21, the classroom was getting ready to go outdoors. Ms. Todd asked Child A to go potty. Child A went to the restroom located inside the classroom. Ms. Todd was standing about five feet away from Child A who was standing outside of the bathroom door. Ms. Todd saw Child A quickly retract her hand and pull it to her chest. She asked her what happened, and Child A told her she shut her hand in the door. Child A was calm and not crying. Ms. Todd saw a lot of blood and quickly guided her to the handwashing sink located in the classroom, outside of the bathroom. She ran water over Child A's hand while verbally comforting the child. She quickly wrapped paper towel around it and applied pressure. She feared Child A's sight of the blood would upset the child. Another staff member, Marilyn Drake, was called into Ms. Todd's classroom to supervise the children while Sayeda Baset, the second staff member in the classroom, left to get ice. Ms. Todd called Child A's mother and told her the cut must be serious due to the amount of blood. She offered to call 911. The parent declined and said she would be there within five minutes. Ms. Drake attended to Child A as she waited to be picked up at the door. Ms. Todd returned to the classroom for the child's backpack. She then checked the bathroom for blood. She only found a drop, then saw the tip of Child A's finger on the floor. She wrapped it in ice. Child A's mother arrived within two minutes to take Child A to the hospital. Ms. Todd gave her the severed finger. Ms. Todd knew Child A had a serious cut given the amount of blood but was surprised to see the severed finger, especially given Child A's calm demeaner. Child A never cried. Ms. Todd said the restroom door where the incident occurred is located within the adjoining classrooms, rooms 5 and 6. Children often tell staff they are going to use the restroom and enter and leave on their own. Ms. Todd said she uses the doorstop to keep the bathroom door open about two to three feet. She said there is no specific policy regarding this practice, but most children like it open, and many are afraid to be inside alone. Some children want privacy and close the door. Child B is believed to be the child in the restroom changing her clothes at the time the accident occurred. Ms. Todd thought this was the case since she later noticed Child B's bag of clothing in the bathroom. Child B often closes the bathroom door. She is fully toilet trained but frequently wants to change her clothes when she senses dampness. Ms. Todd speculated that the door may have been partially open at the time of the incident and Child A likely grabbed the outside of the door, rather than the outside handle, and that Child B had pulled it shut on her finger. Ms. Todd and Ms. Baset were the two staff members present in the classroom when the incident took place and have worked in the classroom since the start of the school year. All children in their adjoining classroom are at least three years old with no more than eight children on each side of the room or a total of 16 children at any given time. 3 Licensee designee Lisa Gryglak indicated 16 children were present when the accident happened. Ms. Todd was unaware of any other injuries related to the bathroom door. Prior to my visit to the center, I interviewed Child A's mother on 11/18/21. She said her daughter's bandages are off and that she is healing well. Her understanding of the incident is that her daughter put her hand in the door and another girl shut it on her hand. Her daughter told her this. She believes Ms. Todd immediately called her about the accident. It did not sound serious. She believes she heard something about it being bent or an injury at the top of her finger. She was asked if 911 should be called and said no. She only lives four minutes away. She was not prepared for what she saw when she arrived. Staff use the word "sever" and she does not believe the

The premises was not maintained in a safe condition that did not pose a threat to safety. Bathroom doors with self-closing features did not fully close. Often, they remained open approximately six inches to two feet. In Child A's classroom, previous staff members had modified the bathroom door by adding an adhesive interior handle which gave children who wanted the bathroom door shut for privacy the ability to pull it shut. Adding this handle would allow children to pull shut the heavy door more quickly than its slow-closing feature was designed to do. Staff reported that children in other classrooms where there were no handles on the interior side of the bathroom door would grab the edge of the bathroom door to pull it shut, sometimes pinching their fingers in the process.
