Child care staff failed to provide appropriate care and supervision of children when multiple children with challenging behaviors repeatedly hit other children, injured children when tossing over shelves, hit staff, repeatedly ran out of the classroom and were taken to the office when the behaviors were unmanageable.
On November 17, 2025, Lead Teacher 1 was witnessed squeezing the cheeks of Child A while telling him "you don't speak to me that way."
All of the infant's bottles in Infant 2 were uncovered. One infant's bottles was uncovered in Infant 1.
CONCLUSION: REQUIREMENT NOT MET CORRECTIVE ACTION PLAN: All staff will use gloves and utensils when serving food. DATE COMPLIANCE WILL BE 10/7/25 ACHIEVED BY: Rule/Statutory Violations
Children had beverage containers including bottles in both infant roomsand the Toddler 2 room.
During the 09/10/2025 onsite inspection, a milk container in the Infant 2 room had a July date and a September date written on the carton. The milk had expired on 09/09/2025.
During the onsite inspection. cots were spaced less than 18 inches apart in a number of rooms.
During the onsite inspection, children ate in the school cafeteria. The tables were sprayed with a one step multi- sanitizer. The table was not cleaned or rinsed before the sanitizer was used.

A fire drill was conducted in November 2024 then not again until March 2025.
