During a review of emergency forms, it was found that a two year old boy has not had his emergency form updated. It was last signed by the parent on 06/23/2024. Emergency forms must be updated at least on a yearly basis. Please have the parent review the form for accuracy, initial and date the form for the yearly update. Please send a copy of the updated form to OCC for the correction of this non-compliance.
During a review of children's records, it was found that a 1 year old girl needs a blood lead test and a MMR vaccine and a Varicella vaccine. This may be missing due to her having only just turned one year old approximately two weeks ago. Please inform her parent of the missing blood lead test and vaccinations so that she may obtain them and provide documentation to you. In addition, a 1 year and 4 month old girl is missing her Health Inventory Part I and another DTP vaccination. Please inform her parent of this missing documentation and immunization so that the parent can complete, sign and date the form and return it to you as well as obtain another age-appropriate vaccination for her child or the documentation of already having it. A two year old girl is missing documentation of any blood lead testing. Please inform the parent of this so that they may either provide you the results or schedule her test and provide the results. After receiving the documentation, please keep a copy for their file and send a copy to OCC for the correction of this non-compliance.

There is no evidence that the Provider has taken MSDE's 2024 Basic Health and Safety Update that was due no later than 12/31/2024. Please find your certificate of completion and send it to OCC or re-take the course and send the completed training certificate to OCC for the correction of this non-compliance.
The Provider was late in completing her training for her 12/01/2023 to 11/30/2024 training year. She has since made up all of the required 12 hours of continued training as of 02/06/2025. Therefore, this non-compliance has been corrected.
During a review of children's emergency forms, it was found that two children's emergency forms are missing all of their physician's information and two other children's emergency forms are missing their physician's address. Please notify the respective parents so that they may add the missing information, initial and date the form for the update. Please send a copy to OCC of the updated emergency cards to include the missing information for the correction of this non-compliance.
During the inspection, a pillow and a thick fluffy blanket were found in two of the three pack-n-plays. Licensing Specialist educated the Provider about the dangers of such objects as they are a suffocation hazard. The Provider removed all of the items from both of the pack-n-plays during the inspection. Therefore, this non-compliance was corrected on-site.
