Cited finding
During a sampling of children's records, it was found that one child's emergency form is missing the physician's address and phone number. Another emergency form is missing the child's address and the physician's address. Please notify the respective parents so that the parent may add the missing information, initial and date the form to indicate the updated information. Please send a copy of both emergency forms with the added information to OCC for the correction of this non-compliance.
