The children who are missing Part I of the health inventory are also missing the individual needs information for them. Once Part I of the health inventory is received for them, this will be corrected.
The gate at the bottom of the stairs needs to be fixed. Right now it won't remain closed.
Provider no longer has the hard base of one of the pack n plays. She states that a child was sick on it and couldn't get it cleaned well. The provider has placed a soft mat in the bottom of the pack n play and a 13 month old child was napping on it at the inspection. Provider may not use this pack n play without the proper base in place. Provider took the hard base from her other pack n play and put it in the other one. The soft mat was removed. Provider is going to have to get another hard base or a new pack n play in order to remain in compliance with having two pack n plays for her registration. She needs to send me pictures of two appropriately equipped pack n plays.
The 13 month old child was sleeping on a boppy pillow. There shall be nothing in a pack n play with a sleeping child. The child woke up and provider was told that the boppy may not be used in the pack n play.
When specialist arrived the provider's 18 year old daughter was with the sleeping 13 month old. The provider had run upstairs to get the children's records for the inspection. The provider's daughter is not an approved substitute and cannot be put in place to supervise any children. This was corrected when the provider returned to the child care space within one minute of the specialist arriving.
Emergency plan was not updated. It was corrected during the inspection.
The first aid kit needs tweezers and 2 rolls of gauze.
3 children need their emergency cards updated.
There was a basket containing Lysol wipes and cleaners down at floor level. These were moved up out of reach during the inspection.
