LS observed that one child is required to have part II of the health assessment and one child is missing Immunizations.
While measuring the black top, LS observed that the lower part of the fence on the far right side of the playground was detached from the ground and sticking upward and out into the approved playground area. LS will complete follow up in a week. Blacktop is restricted from play to prevent children from injury, until after the follow up.

LS observed that one child is required to have the emergency form completed.
LS observed an employee who is unknown to the OCC. LS inquired how long he has been at the center. Per employee, , who stated he is he is the driver, has been with the center for at least 2.5 years. LS confirmed he has been employed at the center since 2023.
LS observed unknown adult supervising a group of children. This employee has not been approved by the OCCC to supervise a group of children in care. Director arrived approximately fifty (50) minutes later. Corrected on site.
LS observed employee who has not been qualified as a Child Care Teacher supervising the children in care. LS arrived at 7:30a and observed unqualified staff with the children for approximately fifty (50) minutes. Director arrived at the center at 8:20am. Corrected on site
Unknown employee, who is a designated driver, was observed driving the children. Employee does not hold first aid/CPR certification. Director informed LS that they are schedule to take first aid/CPR next week. LS advised Director to have unknown driver take the course as well.
This licensing inspection recorded no violations or advisories.
